Position Summary
The UNC Children’s Complex Care team serves about 300 children and families from across North Carolina, primarily in the Piedmont Region in the cities and counties around Raleigh, Durham, and Chapel Hill. Our team provides clinical care and care coordination services across the spectrum from hospital to community. Our goal is to help families and clinical care teams provide more efficient, effective, and family-centered care that optimizes quality of life and well-being for children with complex needs and their families. We are seeking to incorporate a team member with lived experience of raising and caring for a child with complex needs. This position (a) will serve as a professional peer resource for families in our Complex Care Program and (b) will be a liaison between families and representatives of clinical and community services on which they rely.
Provide direct patient and family service as a member of the UNC Children’s Complex Care team by linking families to resources at Children’s and in the community. Interact with patients, parents and other family members, physicians, and other staff under moderate supervision in a courteous manner. Maintain referral data base and assist in generating monthly reports about contacts with families and outcomes of support provided.
Minimum Education and Experience Requirements
Associate’s degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.
Required Qualifications, Competencies, and Experience
The successful candidate should have related experience, which may include experience as employee in a medical setting or as a patient or family member in the health care system. Strong organizational and communication skills. Computer knowledge including Microsoft Word and Excel.
Preferred Qualifications, Competencies, and Experience
Lived experience as a parent or guardian of a child (preschool-age or older) with complex medical needs strongly preferred.
Familiarity with UNC Health System and community resources for children with special health care needs preferred. Spanish language skills are highly desired for the individual in this position. Our program serves many families who are primarily speakers of Spanish.
