Registered Nurse (Experienced RN)

Peds Medicine & Rehab (PMR) Nurse Navigator

Located in Children’s Health Center

Department Profile:
The Pediatric Medicine and Rehabilitation (PMR) Service at Duke Children's is active in delivering exemplary care for each patient to ensure an excellent patient experience.

This position will provide triage support for this patient population. This position will work closely with the PMR faculty.  We are looking for someone with the ability to establish a strong rapport with patients and families by phone, coordinate services, and provide education and nursing support to patients and families in and out of the clinic setting.  Work environment is a combination of independent work alongside team collaboration and is a supportive, dynamic and challenging environment.
The ideal candidate for this position will possess the following:

  • A solid background in Pediatric Nursing
  • Experience with telephone triage or managed care coordination
  • Strong oral /written communication skills, software navigation abilities (EPIC experience preferred)
  • Good organizational skills
  • Flexible attitude with the ability to function both independently and as a team member, assisting patients/ families through home care coordination and in the clinic setting
  • Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others

  • Work Performed (but not limited to)

    • Triage calls resolving problems within scope; gather relevant information and coordinate follow-up with providers to develop plan and convey plan back to patients.  Document communication in Maestro care and help manage MyChart messages as appropriate. 
    • Assisting with medication refills, referral management, completion of needed forms (DME, school, DMV), etc.
    • Assist in coordinating diagnostic testing and outpatient appointments here at Duke or at  outside facilities
    • Patient and family education on plan of care for procedures, therapies, and equipment
    • Assist social work on transitional care for patients moving to adult based care

     
    Work Schedule:

    Full Time: Hours may vary, but will typically fall within a standard Monday-Friday, 8am-5pm schedule.

    What you will do:

    As a Nurse Navigator in Duke University Health System, you will plan and provide professional nursing care for patients in accordance with the medical and nursing plans of care and established policies and procedures. Provide nursing services to patients and families in accordance with the scope of the RN as defined by the North Carolina Board of Nursing.

    • Will attend clinic as needed to provide clinic coordination and patient education
    • Patient care coordination will be done in an office setting on non-clinic days
    • Triage calls resolving problems within scope; gather relevant information and coordinate follow-up with providers to develop plan and convey plan back to patients.  Document communication in Maestro care and manage in-basket messages regarding care. 
    • Patient care coordination involves infusion management, assisting with refills, entering labs/studies needed during appointments, triage patient phone calls, coordinating various appointments/studies, obtaining prior authorizations for medications, assisting with various medical/school forms, etc.
    • Coordinate new patient appointments assuring all necessary medical records have been obtained from outside facilities
    • Coordinate diagnostic testing and outpatient appointments here at Duke, or at an outside facilities

    What you can expect:

    • Multitask in myriad ways – document assessments, plan of care, interventions, evaluation and re-evaluation of patient status, access nursing needs of acute and chronically ill patients, independently seek out resources and work collaboratively. Educate patients and families in accordance with the nursing plan of care. Relationship management with patients, families, visitors, healthcare team, physicians, administrators, leadership and others.

    What you will need:

    • Graduation from an accredited Bachelor’s Degree in Nursing (or higher) OR Associate's Degree in Nursing OR Nursing Diploma program.

    ***All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date.

    Licensure and Certification Required:

    • Current registration with North Carolina State Board of Nursing as a registered professional nurse OR current compact RN licensure to practice in the state of North Carolina required.
    • BLS Certification required
    • PALS (Preferred)
