Job Summary: Key Job Responsibilities
The Diabetes Educator, as a professional practitioner, assumes responsibility for coordination of the hospitalwide
Diabetes Management Program. Serves as a resource, role model, and consultant for all departments and
staff regarding diabetes management issues, promoting hospital wide quality diabetes management. Responsible
for collecting, compiling and interpreting data on hospital diabetes management audits and preparing reports to
committees. The Diabetes Educator develops, coordinates, delivers and evaluates diabetes management
activities for hospital staff, patients and family members. Demonstrates critical thinking and performance ability
in the coordination, implementation, documentation and evaluation of patient care. Maintains competency skills
including the ability to provide care to special patient populations and patients with diverse cultural backgrounds.
The Diabetes Educator leads an outcome-based approach to patient care by insuring evidence-based practice
and interdisciplinary care delivery. Identifies safety concerns, and collaborates with all members of the
healthcare team to institute measures to ensure a safe and timely discharge.
The scope of responsibility, duties and accountability at the facility department and position levels including
knowledge and practice of the clinical/technical skills necessary to the position consistent with the Nurse Practice
Act. Nursing job duties are to meet the Nurse Practice Act (state law that protects patients by promoting safe,
competent nursing practices), Title 22 (Health and Safety codes), The Joint Commission (develops performance
standards and accredits programs for hospitals and other healthcare-related organizations) and the Plan for
Provision of Nursing Care and the Plan for Provision of Care to Patients, and other regulatory requirements.

Primary Duties and Responsibilities
??? Conducts diabetes education to staff, patients, and families including diabetes medications, orals and
injectable, insulin pumps, continuous glucose monitors, and other education needs
??? Conducts and participates in research focused on diabetes management across varied patient populations.
??? Develops educational programs and materials related to diabetes management to meet the needs and
interests of patients, families, nurses, staff, physicians and the community.
???
Participates in community outreach related to diabetes prevention and self- management. Serves as an
advocate and liaison for patients, families, and staff regarding diabetes management including diabetes
survival skills and self-management.
??? Participates in the development and implementation of performance improvement activities, both clinically
orientated and related to the administrative process.
???
Identify the ability of the patients to follow directions/ instructions and give consent. Reduces the risk of
health care acquired infection by complying with current CDC hand washing guidelines and CSMC isolation
policy.
???
Identify safety concerns and institute measures to provide a safe environment; and provide appropriate
assessment, intervention and evaluation of patients with consideration of developmental milestones in the
patient population
??? Participates in house wide committee work in area of responsibility: Member of Patient and Family Education
committee and/or Education Counsel.
??? Develops, coordinates, implements and evaluates educational programs/ projects for patient care personnel
including nursing staff and assigned members of the support staff. Serves as a preceptor for other Clinical

Utilizes a systematic approach to assess educational needs in collaboration with all levels of nursing and
medical personnel and evaluates the effectiveness and efficiency of educational programs. Collaborates
throughout the Cedars-Sinai Health system (CSHS) in initiating and implementing educational programs.

Coordinates programs, projects and events with Nursing, medical center departments and communities
across the health care continuum. Serves as a resource and consultant for educational programs throughout
the Cedars-Sinai Health System. Provides orientation and training for Nursing and health system personnel
???
Ensures compliance with regulatory requirements, including JCAHO, HFCA and DHS. Reduces the risk of
health care acquired infection by complying with current CDC hand washing guidelines and CSMC isolation
policy.
??? Assist with validation of Electronic Health Record (EHR) components regarding best practices, discharge
education and other documentation as needed.
??? Provides and documents patient, family and significant other education, including smoking cessation,
pneumococcal vaccination and other patient specific subjects, utilizing effective teaching methods
??? Improves the safety of using insulin pumps: Medication with clinical indicators will be administered using an
insulin pump

Job Qualifications:

Bachelor's Degree in Nursing - Required

3 years in Acute care experience - Required

California Registered Nurse License - Required

Basic Life Support - Required

Cert Diabetes Educator Nurse - Required
