Department Profile:

The Cerebral Palsy and Related Conditions Clinic (CPRC) at Lenox Baker is dedicated to providing excellent coordinated care to our patients.  This is a multidisciplinary clinic that meets weekly.

This position will provide triage support and care coordination for patient populations with Cerebral Palsy and related conditions, in collaboration with dynamic multidisciplinary teams.  The CPRC Coordinator will serve as the point of contact for this patient population to coordinate and manage their complex medical needs.  We are looking for someone with the ability to establish a strong rapport with patients and families both in and out of the clinic setting, coordinate services, and provide education and nursing support.
