The Department of UHT Social Work is currently seeking a full time Discharge Planner to work at the University of Miami Hospital. This position functions as Discharge Planner for the Case Management Department under the direction of the Director of Case Management. Discharge Planner will coordinate all discharge needs for the hospital discharges. The Discharge Planner will process the physician order based on the patient’s choice and based on insurance contract agreements with certified agencies. Coordination of all Home Health, Durable Medical Equipment, Orthopedics and Infusion.

CORE JOB FUNCTIONS                                                                                           

Processes the physician order based on the patient’s choice and based on insurance contract agreements with certified agencies.

Coordinates all Home Health, Durable Medical Equipment, Orthotics and Infusion.

Utilizes electronic referral system to send referrals for all post discharge needs to licensed providers.

Assures follow-up with companies for the coordination of benefits and services.

Obtains any required forms for completion with the physician attending, qualified practitioner or ordering physician for completion.

Assists the social worker with obtaining patient choice when warranted.

Reviews Medical Record electronically to obtain appropriate documentation for post discharge needs and coordinate services.

Communicates the finalization, delivery, and acceptance of case to the nursing and care coordination staff.

Adheres to University and unit-level policies and procedures and safeguards University assets.
