Scope of Position The Director Care Management, Outcomes and Clinical Documentation Improvement has responsibility for the direction, planning, organization, implementation and evaluation of the inpatient and outpatient James PCRM programs, ESR Outcomes Managers, Clinical Documentation Integrity program, the James Line and After-Hours Clinical Call Center. Ongoing collaboration with other patient care departments/areas that relate directly or indirectly to patient care operations and services is essential.

Position Summary The Director leads the James Patient Care Resource Managers in providing patient care and supportive services directed towards achievement of positive patient care outcomes, maximizing the patient’s involvement in his/her own plan of care, improving service levels, reducing the length of hospitalization, and decreasing costs. The Director is responsible for coordinating case management, utilization management and clinical documentation enhancement activities and collaborating with the OSUHS Case Management Department and Revenue Cycle as well as other James departments. The Director also coordinates ESR outcomes activities, James Line Nurse program and all call center activities. The Director develops and aligns programs with the James value-based care and population health strategic direction.

Minimim Quals

For Hire: Master’s degree required. RN licensure required. 5 + years hospital case management experience required; prior demonstration of leadershipskills in progressively responsible management positions preferred. Care coordination, utilization, case/disease management experience required. Data analytics and reporting experience preferred. Experience in Value Based Care models, managed care, customer service, and provider relations preferred. Proficiency with Microsoft Office applications, specifically Excel and PowerPoint required. Ability to build and foster relationships inside and outside the organizationSignificant understanding of the care continuum and health care provider organizations – acute and post-acute care facilities and providers.Experience in project management.Solid time management/problem solving skills.Experience working effectively with a wide range of colleagues, from entry level to senior executives, physicians, nurses, administrative and information technology staff.Strong verbal and written communication, organizational and human relations skills, self-motivated, professional.
