Scope of Position  

Provision of social work services to inpatients, outpatients and their families in The Ohio State University James Cancer Hospital.

Position Summary:   Social work services include psychosocial assessments, mental health counseling, distress screening, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources and linkage with community agencies.  Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery and safe transition from one care environment to another. Core competencies include:  Psychosocial assessment, counseling and crisis intervention skills, knowledge of the PASAAR process for extended care facility placement and the assessment and counseling processes involved, knowledge of hospice referral process and the assessment and counseling processes involved, knowledge of guardianship procedures, and the skills and knowledge to assess neglect/abuse, including substance abuse and domestic violence.

Duties and Responsibilities  


Care Facilitation: 60% of time

Conduct psychosocial assessments, supportive care screening, evaluation, counseling and interventions for patients and their families to assist them in coping with:

Medical diagnosis resulting in acute and chronic illness

Disability resulting from the diagnosis or pre-existing disability

Terminal condition

Death

Violence and maltreatment including domestic assault, child and elder abuse/neglect

Substance abuse

Relationship problems

  • Mental Health problems
  • Environmental problems including financial, housing, transportation, and activities of daily living.
  • Vocational/educational concerns.
  • Provide services across the continuum of care, including the provision of home health care, agency services, medical equipment, rehabilitation, skilled nursing facility placement, long term care placement, hospice and referral to mental health agencies/hospitals.
  • Facilitate Advance Directives and assist with health care power of attorney, living will and guardianship procedures.
  • May provide transplant evaluations.
  • Provide educational support to assist patients and families to understand and comply with treatment recommendations. 
  • Demonstrate an understanding of the impact of life threatening illness or injury on patients of all ages, and their caregivers and significant others.

·Teamwork:  30 % of time
·May participate in interdisciplinary rounds, patient care conferences and team meetings.
  • May participate in the development of pathways and care plans.
  • Work collaboratively with PCRM to insure comprehensive care.
  • Serve as consultant to other professionals on health care team on psychosocial issues and community resources.
  • May participate in staff meetings, in-services, continuing education programs, research activities, CQI and performance improvement activities.


Documentation:  10% of time

  • Maintain required documentation including medical charting, statistics and records, correspondence as required.

Practices within The Ohio State University James Cancer Hospital policies and procedures.  Adheres to Intensive Caring Values demonstrated through positive patient/guest relations, positive and effective interactions with staff, and formulating and meeting departmental and personal developmental goals. Minimum Qualifications

For Hire:  Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work Licensure (LSW or LISW) or equivalent education/experience.           

Ongoing:  Meet mandatory education and health surveillance requirements.  Demonstrates competence in technical, interpersonal, and cognitive skills required to meet essential job functions.
