The Community Health Worker (CHW) is a frontline public health worker who is a trusted team member with an in-depth understanding of the community served. The OSU Total Health and Wellness (THW) primary care clinic, a Federally Qualified Health Center, and the MOMI PODS program is adding a CHW to our team to serve as a link between health/social services and the community to better facilitate access to services and improve the quality and cultural competence of these services. The CHW builds individual and community capacity and awareness by increasing health knowledge and self-sufficiency through a range of activities, such as outreach, community education, intensive case management, social support, and advocacy. The CHW assists patients in accessing appropriate resources, coordinates referrals as necessary, provides community and educational support, and serves as a community liaison. The CHW will assist in co-leading educational sessions once a month for the patients enrolled in MOMI PODS alongside a Registered Nurse Care Coordinator. The CHW will establish and optimize CHW workflows for the MOMI PODS program in partnership with core team members and clinics.

Required Qualifications: High School graduate or equivalent. Community Health Worker certification. 1 year of relevant experience. 

Desired Qualifications: Must be compassionate and competent in working with diverse cultural, religious, and socioeconomic groups. Must possess strong ability to serve as a community liaison between target populations, agencies, providers and team.
