Primary Purpose of Organizational Unit | The UNC School of Medicine has a rich tradition of excellence and care. We optimize our partnership with the UNC Health System through close collaboration and commitment to service. This position is based at the UNC REACH Enhanced Primary Care clinic. UNC REACH, formerly WakeBrook Primary Care, is an enhanced primary care clinic dedicated to meeting the primary care health needs of North Carolinians with severe mental illness, and has been in operation since 2015. Nearly 100% of clients served through the clinic have lived experience of severe mental illness, substance use disorder, incarceration, homelessness, and/or disability. The clinic has earned the Advanced Medical Home Plus (AMH+) designation under DHHS and NC Medicaid. Enhanced primary care goes above and beyond typical primary care, providing specialized provisions and staff training related to severe mental illness. The clinic also offers medications for opioid use disorder (MOUD), a hepatitis C treatment program, psychosocial groups, peer support, and dental clinic through the UNC School of Dentistry. The FIT Program is a partnership between UNC Family Medicine, the North Carolina Department of Adult Correction (DAC) (which administers the state prison system), Federally Qualified Health Centers, County Departments of Health, community-based organizations, and local reentry councils. The FIT Program coordinates with existing state and local resources to assist people coming out of incarceration with connections to essential medical services. The program hinges on Community Health Workers (CHWs) who connect eligible clients with chronic disease, mental illness, and/or substance use disorder to appropriate health care services. FIT CHWs also help clients put together comprehensive reentry plans working with local reentry partners and community resources. Similar programs have been successful in other parts of the U.S. and have decreased rates of hospitalization, emergency room use, and even recidivism. We have additional support from a national group that encourages the use of this model, the Transitions Clinic Network. |
Position Summary | The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to patients recently released from prison into Wake County with a diagnosis of a serious mental illness (for example, schizophrenia, schizoaffective disorder, or bipolar disorder) in addition to chronic medical conditions and/or substance use disorders who are referred by prison health providers prior to prison release. Patients will receive psychiatric and primary care services at the UNC REACH (Recovery and Access to Comprehensive Health) Enhanced Primary Care clinic. |
