Summary

The Community Health Worker (CHW) will help increase access to health services for adolescents and young adults (AYA) in the community through outreach, education, community engagement, partnership engagement/coalition, and peer and patient support. The CHW is expected to understand AYA and sexual health related issues and be willing and ready to engage with clients who struggle with these issues at all levels. The CHW is responsible for assisting patients in linking them to appropriate health care services through existing and new partnerships. Additional job duties include planning preparing strategies with community based medical and social services, providing basic patient/client advocacy, assessment, planning and casework services, strengthening existing and developing new relationships with area social service agencies to build knowledge of the resources available to patients, participating actively in regular supervisory and team meetings, training sessions, conferences, seminars, program and operational planning, and working towards developing a broad knowledge of casework. This position will also help develop and strengthen new and existing partnerships, participating in the Youth Advisory Council and community partner coalitions, improving the patient satisfaction survey, increase patient linkage to prevention and care services for co-occurring conditions, training staff on referral options and processes, and helping patients navigate and successfully link patients to prevention and care services for co-occurring conditions not available at BTHC.

 

The Baylor Teen Health Clinic currently provides pregnancy testing, family planning, sexually transmitted diseases (STD) and HIV/AIDS education and screening, primary healthcare, and case management services to teens, their partners, and adolescents in Harris County. The clinics provide services to teens from the age of 13-24. This position may require evenings or weekends as needed.

Job Duties
  • Conducts outreach and prevention education services, targeting individuals in need of a medical home and/or with chronic disease.
  • Plans and prepares strategies with community health center and local sites about referral process.
  • Assists clients in gaining access to and navigating a primary health care medical home and other community based social services (i.e. behavioral health services, housing, legal, etc.).
  • Documents each patient served, conducts intake interviews, monitors client progress, maintains data collection logs and form.
  • Visits clients to develop relationships and promote harm reduction.
  • Provides basic patient/client advocacy, assessment, planning and casework services. 
  • Provides culturally and linguistically appropriate services and health education to clients. 
  • Maintains a professional disposition while working with a multidisciplinary health care team. 
  • Coordinates transportation for clients to/from medical appointments, including accompaniment as needed.
  • Completes accurately, and in a timely manner, all-necessary forms, case notes, and reports, and submits such documentation to his or her supervisor within designated timelines. 
  • Develops relationships with area social service agencies to build knowledge of the resources available to clients.
  • Participates actively in regular supervisory and team meetings, training sessions, conferences, seminars and independent study. Participates in program and operational planning. 
  • Works toward developing a broad knowledge of casework and carries the volume of work as determined with his or her supervisor.
  • Follows-up with clients regarding appointments, care plans, and health goals.

Minimum Qualifications
  • High School diploma or GED.
  • Two years of relevant experience in health/social services.

Preferred Qualifications
  • Bachelor's degree in Social Work
  • Experience with online survey tools is a plus.
  • Experience with youth engagement is a plus.
  • Versed in the Microsoft Office Products.
  • Experience with data analytics is a plus.
