Job Description Summary:
The Community Health Facilitator (CHF) will primarily work in the community with disparate populations in Wichita to help prepare individuals to navigate health care and social systems by connecting them to resources. The CHF will also facilitate community health educational sessions focused on improving health literacy. Educational sessions will include but are not limited to healthy behaviors, mental health, health visits and screenings and COVID-19 prevention.
The CHF will connect disparate individuals and families to resources to improve health outcomes. The CHF will work closely with community partners and other agencies to identify gaps in services to ensure cultural and linguistic appropriate services, according to National CLAS standards.
This position will serve as a support and navigator to individual patients, clients, and community members through health and social service systems. A CHF will serve as a liaison between the community members and the health and social service agencies.
This position will support the Facts Not Fear ICT project, which aims to advance health literacy, incorporate Wichita communities in all aspects of the work, empower health care providers to effectively communicate with patients, and increase vaccination rates for target communities. Facts Not Fear ICT ultimately aims to increase positive health outcomes for disparate communities in Wichita, Kansas. Specifically, Facts Not Fear ICT focuses on Hispanic/Latino, Black/African American, Asian, and American Indian and Alaska Native communities in Wichita. Community Health Facilitator (CHF) duties will include: team strategizing and planning with project team to address social determinants of health barriers through advocacy, support, and education. CHFs will also work to improve COVID-19 vaccination and testing efforts in these communities through community outreach and events, and participant recruitment.
This position is full-time, 40 hours per week. We have 4 openings for this position.
This position will pay $21.64 hourly, commensurate with experience.

Job Description:

    Required Qualifications:

    • Education:
    • Bachelor’s degree or equivalent combination of education and relevant experience working in community-based projects
    • Work Experience:
    • Experience using email, internet, and Microsoft Office Products (Word, Excel, and PowerPoint)
    • Experience working with underserved and diverse populations in Wichita community
    • Skills:
    • Flexibility to work some nights and weekends, hours can be flexed within same week
    • Knowledge of health disparities and community needs among Black or African American, Asian, Hispanic or Latino, or American Indian or Alaska Native community in Wichita


    Preferred Qualifications:

    • Experience:
    • One year of experience working as a community navigator or related role
    • Received Community Health Worker training
    • Skills/Competencies:
    • Being bilingual in Spanish or Vietnamese would be beneficial for 2 of the positions
    • Familiarity with community and health care systems and their resources
    • Ability to effectively organize and concurrently manage multiple tasks
    • Ability to approach problems using critical thinking skills
    • Self-directed and able to exercise independent judgment
    • Excellent written and oral communication skills

    Job Duties Outlined:

    • Support other Community Health Facilitators in serving the Hispanic/Latino, Black/African American, Asian, and/or American Indian or Alaska Native communities in Wichita
    • Assist in planning and development of community events to reach project goals
    • Engage and recruit community members to participate in project activities
    • Implement best practices surrounding health literacy and COVID-19 prevention
    • Engage with community to assist in identifying barriers to health and addressing individual and community needs
    • Make referrals or address other barriers to healthcare access such as language barriers, transportation needs, navigating the healthcare system (e.g., making an appointment, helping prepare for appointments, transportation to appointments, helping facilitate conversations, adherence plans, medications, identifying resources for payment) 
    • Facilitate educational sessions about the importance of receiving health services, including health screening
    • Maintain and review an up-to-date inventory of community resources and services to share with community members as needed
    • Assist in project reports and presentations about Facts Not Fear ICT
    • Assist project evaluation efforts in recruitment and data collection at community events
    • Work directly with project team and partners 
    • Meet with Project Assistant to report progress toward project goals
    • Complete required trainings including Community Health Worker training
