THIS IS A GRANT FUNDED POSITION LOCATED IN SAVANNAH, TN

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: The Community Health Coach will be responsible for coaching, motivating, and supporting patients in the Equitable Health in Neighborhoods (EHiN) Program. This position engages participants in physical fitness and nutrition activities to improve health outcomes and manage or prevent chronic diseases related to obesity.

 

DUTIES AND RESPONSIBILITIES

  • Establishes trusting relationships with participants to encourage engagement with individualized programs and healthcare providers for improved health outcomes.
  • Assesses and addresses barriers to access to preventive care.
  • Collaborates with medical professionals and program leads to create educational materials for participants.
  • Educates program participants on disease prevention and advocacy for improved health care.
  • Coordinates services with internal and external behavioral and mental healthcare providers.
  • Conducts virtual and/or in-person follow-ups to promote continued engagement and retention.
  • Generates referrals to community support and works with community organizations and rental offices to recruit program participants.
  • Partners together for mini events throughout the community: health-related classes and displays, screenings, etc. 
  • Collects and enters data into the database.
  • Works closely with the UT Extension office staff to develop relationships in the community; aligns activities with the UT Extension office activities.
  • Performs other duties as assigned.

    Qualifications

     

    MINIMUM REQUIREMENTS:  

    EDUCATION: High School Diploma or GED. (Associate’s or Bachelor’s Degree in Health, Social Services, Human Services, or a related field is preferred.) (TRANSCRIPT REQUIRED)

    EXPERIENCE: Two (2) years of related job experience. A valid Driver’s License is required. (Experience as a Community Health Worker or related job specializing in chronic disease management or prevention related to obesity is preferred.)

    KNOWLEDGE, SKILLS, AND ABILITIES:

    • Ability to communicate effectively with program participants, healthcare providers, and community partners.
    • Ability to provide effective education and instruction to program participants.
    • Ability to assess the needs of participants and respond appropriately.
    • Ability to influence and coach individuals.
    • Ability to work in Microsoft Office Suite and virtual platforms.
    • Knowledge and experience working with diverse populations, specifically with varying socioeconomic backgrounds.
    • Ability to be self-motivated, tactful, and professional both in-person and virtually.
    • Ability to relate to individuals working to manage or prevent chronic illnesses related to obesity.
    • Excellent interpersonal, verbal, and written communication skills.
    • Excellent organizational and time management skills.

    WORK SCHEDULE: This position is frequently required to work weekends and evenings. Occasional overnight travel is required.
