1. Database building/maintenance
    a. Populate database of all patients seen in MLTCR by CRS surgeons
        i. Maintain NAPRC compliant sub-database of all primary rectal cancer patients

2. Patient Care Coordination/Navigation
    a. Coord Care within and outside Rogel Cancer Center 
    b. Communication with outside providers offices
    c. Triage clinical patient questions to nurses/PAs/physicians
    d. Ensure timely initiation of care after NP visit

3. Work with Rogel intake personnel 
    a. Assist with getting outside records
    b. Ensure timely treatment starts
    c. Ensure timely imaging/other testing/referrals
    d. Ensure compliance with NAPRC documentation standards

4. Patient tracking
    a. Active treatment patients
        i. Surgery ensure all patients who have surgery are updated in database, and presented at TB prior to surgery and after surgery
    b. Watch and wait patients (WW)
        i. ensure all WW patients are tracked in database
        ii. ensure all WW patients are presented at TB prior to WW decision 
        iii. ensure surveillance/compliance with imaging/endoscopy and labs for all WW patients

5. Measuring care standards for every NP and surgery patient
    a. Ensure all TB presentations occur

6. Tumor board
    a. Attend every week
    b. Tumor board plans to be delegated to coordinator to work with relevant providers to make sure plans carried out
    c. Complete tumor board notes for colorectal surgery patients

7. Quarterly program audits
    a. Chart reviews, and creation of performance reports
        i. All new RC patients 
        ii. All surgical RC patients
        iii. All watch and wait RC patients

  • Bachelors with clinical experience, or graduate degree (e.g. MPH)
  • Detail-oriented
  • Organized 
  • Comfortable calling/speaking with patients
  • Comfortable speaking with physicians/physician offices
  • Must have experience with Microsoft Excel, Microsoft Word, Microsoft Outlook, and database familiarity (Microsoft Access, SQL, etc), to track patients
