Primary Purpose: 
The Primary/Ambulatory Care Pharmacist will work with the healthcare providers in the UC Health – UCPC Primary Care Network which has locations across the Greater Cincinnati area and the Pharmacy Department to provide the following services:


  • Practice will build around: Diabetes management, hypertension, hyperlipidemia, COPD/asthma, medication adherence, medication review, transitional care management, congestive heart failure, and anticoagulation management.

  • General and patient specific drug information to patients, physicians, nurses and, and other health care professionals.

  • Provides pharmaceutical care to patients by direct interaction, and formal educational programs.

  • Supports and participates in pharmaceutical related research in area of specialty.

  • Maintains expertise in areas of practice through continual learning, journal review and other educational activities.

  • Educates pharmacy residents, technicians, pharmacy students, allied health professionals, nurses and physicians in pharmacotherapy within in the specialty area.

  • Coordinates and provides the pharmaceutical care of patients for an assigned clinical unit and participates in patient care rounds with medical staff personnel.





Qualifications

 

Hours:  Full Time, Monday-Friday
Education: Master's degree, minimum required. PharmD, preferred degree.

  • Completion of an ASHP-accredited PGY1 pharmacy residency, with preference for ASHP accredited PGY2 and/or fellowship in specialty area.
  • Significant research contributions and medication use evaluation (MUE) activity in specialty area of practice, preferred.
  • Recognized as an expert in field of specialty (locally, regionally, or nationally) through professional service, presentations, and/or publications.

License and Certification: Registered pharmacist or eligible for licensure in Ohio. Current certification in Basic Life Support (BSL) and Advanced Cardiac Life Support (ACLS).


Experience: 

  • Residency/Fellowship training in primary care or ambulatory care with experience in anticoagulation management is preferred
  • Equivalent experience in a practice setting (3 years)
  • Eligible for licensure within the state of Ohio

Required Skills and Knowledge:

  • Demonstrate and apply in-depth knowledge of pharmacology, pharmacotherapy, pathophysiology, and the clinical signs, symptoms, and natural history of diseases and/or disorders.
  • Demonstrate evidence of exceptional teaching, writing, and communication skills.
  • Development of clinical pathways/medication use guidelines.
  • MUE and project design and data synthesis.
  • Electronic database management.
