Responsibilities*

Participate and assist in the direction, functional supervision and provision of daily home infusion pharmacy operations, including; pharmaceutical patient assessment, care planning and disease state management, clinical drug monitoring, patient and clinician follow up, care coordination with other health care professionals, computer data entry, production, packaging, shipping, quality assurance, and regulatory compliance to ensure safe and effective pharmaceutical care for all patient age groups.


Required Qualifications*
  • Possession of a state narcotics license is necessary. Doctor of Pharmacy degree or Master and/or Bachelor of Science degree in Pharmacy with one year residency or fellowship or equivalent clinical experience.
  • A Certificate of Residency from an accredited program or an equivalent combination of education and experience is necessary.
  • Ability to read, write, speak, and comprehend English.
  • Some experience in clinical pharmacy is necessary. 
  • Some experience in didactic and clinical teaching, including undergraduate and postgraduate level courses, is necessary. 
  • Licensure in good standing, or immediate eligibility for Licensure, as a Registered Pharmacist with an active controlled substance license in the State of Michigan.
  • Possession of a state narcotics license is necessary.

Graduate Non-Certified Patient Care Professional Underfill

  • Minimum of a Bachelor degree in a recognized field of science or learning which is directly related to the duties of the position or an equivalent combination of education and directly related experience

Physical Requirements*
Functional visual acuity and hearing for accurate assessment and recording of patient observations and information in clinical records. 
Able to perform the assigned duties and responsibilities with or without reasonable accommodations: ability to work in a sedentary position for long periods of time, ability to communicate/ exchange information, fine motor coordination/dexterity necessary for keyboarding or typing, and may require reaching, bending, twisting, stooping, moving about, lifting of up to 10 pounds, etc., with or without the use of assistive/adaptive devices.


Desired Qualifications*

Experience in home infusion pharmacy with a demonstrated knowledge of regulations, standards, and guidelines applicable to home infusion pharmacies, admixture practices, and clinical pharmacy services.

Part Time: 24 hours per week

Background Screening
