Duke University Hospital has 1,048 licensed inpatient beds and offers comprehensive diagnostic and therapeutic facilities, including a regional emergency/trauma center; a major surgery suite containing 65 operating rooms; an endo-surgery center; a separate hospital outpatient surgical department with nine operating rooms and an extensive diagnostic and interventional radiology area. In addition to its hospitals, Duke Health has an extensive, geographically dispersed network of outpatient facilities that include primary care offices, urgent care centers, multi-specialty clinics and outpatient surgery centers.
General Description of the Job Class
This clinical pharmacist will maintain a full time hematology/oncology clinical practice in the GU Medical Oncology Clinic.
Duties and Responsibilities of this Level
Clinical Pharmacist Responsibilities
Clinical Practice and Medication Management (80%):
- Participate in GU clinic and provide clinical pharmacy services and oversight for safe use of medication. This includes prescribing, dispensing, administering, and monitoring of medications. This may include, but is not limited to:
- Perform chemotherapy counseling/education
- Complete/Review medication review/reconciliation
- Provide medication counseling
- Interpret pharmacokinetic/therapeutic drug monitoring data
- Screen for drug interactions, therapeutic duplications, omissions, etc.
- Provide initial screen and evaluation of chemotherapy orders for appropriateness
- Monitor patients for therapeutic and/or toxic response to medication/chemotherapy
- Evaluate and address supportive care needs
- Provide oral chemotherapy counseling, adherence tools and follow-up and documentation in case management system
- Coordinate patient care with inpatient colleagues
- Ensures pharmacy staff competency so that pharmacists are providing appropriate drug therapy with consideration of the patient's diagnosis, age, weight, allergies, ADRs, organ function, and diet.
- Provides support to improve safety and to reduce risk to patients treated in the Program. Reports events and/or concerns to SRS system.
- Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and correct utilization of resources to optimize care.
- Provides coordination and oversight of transition of high-acuity treatments from inpatient to ambulatory infusion center.
- Provides leadership of developing guidelines, order forms, and critical pathways to improve the care and safety of patients.
- Provides leadership to maintain TJC accreditation standards within the realm of medication use within the Program.
- Provide guidance and educational support to all pharmacy staff providing support to myeloma patients
- Represent the GU concerns for Information Technology applications.
- Participate in GU grand rounds, tumor boards, patient-care rounds, etc.
- Coordinate with Duke retail pharmacies to ensure medication adherence.
- Collaborates with Cancer Center Infusion Pharmacy staff to provide continuity of care on medication related issues.
Research Practice Management (5%):
- Provide support of clinical trials education of Program staff to assure research compliance.
- Provide education to participants in clinical trials as needed.
- Develop and support study protocol development. This includes the development of concept sheets, in conjunction with the Investigational Chemotherapy Pharmacist, for pharmaceutical-sponsored, investigator-initiated studies and other protocols with myeloma faculty members.
- Develop and participate in pharmacy-related research projects and quality improvement initiatives, MUEs, etc. within the Program.
- Provide oversight of pharmacy staff implementing drug-related clinical research within the Program. When opportunity arises, serves on the IRB, IRB Audit Subcommittee, or CPC Protocol Review and Monitoring Committee.
Educational Practice Management (10%):
- Provide oversight of GU staff education for critical elements of medication use in patient care and clinical research.
- Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of oncology therapies including chemotherapy agents.
- Participates in the education of pharmacy students and pharmacy residents
- Coordinates effort to provide timely drug information to the Program.
- Provides leadership support for research efforts in patient care areas.
- Assures that staff is competent and compliant in providing adequate patient education pertaining to drug therapy within the Program.
- Presents scholarly materials at educational conferences for the myeloma and pharmacy professional meetings and organizations (Duke, State/Local, National)
Administrative Practice (5%):
- Participates in departmental and hospital committees (ie. Oncology Formulary Evaluation Team) and initiatives related to oncology.
- Document clinical activities, consultations, education, etc. in EMR
- Refer patients to patient assistance programs as needed
- Works with peers to achieve division and departmental goals
- Maintains familiarity with all departmental programs and works as integral part of the pharmacy team.

Knowledge, Skills and Abilities
- Ability to communicate clearly in oral and written form.
- Extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states. Ability to work with information systems, automated and technical equipment.
- Ability to establish and maintain working relationships with various members of health care teams
Required Qualifications at this Level
Doctor of Pharmacy Degree OR BS Pharmacy Degree
- With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
- With a Doctor of Pharmacy degree, no experience is required.
- Three plus years of oncology clinical practice experience and/or, PGY2 oncology residency and BCOP preferred, and a demonstrated ability to achieve positive patient outcomes in a similar environment.
- Preferred experience with Epic
- Preferred hospital experience.
Degrees, Licensures, Certifications
- Licensed Pharmacist or eligible to practice pharmacy in North Carolina
