The Transplant Clinic CNC2 is a Registered Nurse with experience and knowledge of intravenous infusions, solid organ transplantation, and living donation.  Transplant Clinic CNC2 rotates to the assignment of need within the transplant department such as the access CNC2 coordinator, transplant CNC2 coordinator nursing staff, and clinic nursing duties.  Performs intravenous infusions as needed. Practicing under the supervision of transplant surgeons and physicians, the Transplant Clinic CNC2 ensures coordination of the clinical aspects of transplant patients and living donor care throughout the infusion process, pre and or post-transplant phase.
Providing the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Transplant Clinic CNC2 provides patient care and consultation, coordinates the educational development of staff members and acts as a role model in clinical practice. The Transplant Clinic CNC2 strives to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice, Relationship Based Care and standards of nursing excellence into clinical practice. Through therapeutic relationships with patients, families, self and colleagues, the Transplant Clinic CNC2 participates in monitoring, developing, and evaluating nursing practice while fulfilling unit and organizational goals.
