Come join our Venice Family Clinic team as a Registered Nurse for Care Coordination. Under the supervision of the Nursing Supervisor and using a multidisciplinary team approach, this RN role will perform care coordination for high-risk high-utilization patients and provide direct nursing services to patients. Using your advanced practice nursing skills, you will: * Identify unmet patient needs and coordinate with PCP to assure that standards of care are met and gaps or barriers to care are addressed. * Perform patient outreach to engage and motivate patients in self-care. * Coordinate care across the continuing, including collaboration with assigned Home Health Agencies. * Provide direct nursing services to patients (adults and children) in our free, community-based primary health care clinic(s) in Los Angeles County. * Prepare patients for medical examination including taking medical history, performing health screening tests, lab tests, giving injections and emergency procedures when necessary. * Provide health education and health assessments. * Triage medical problems, assist with dispensing of medications, and provide nursing consultation. * Participate in programmatic reporting, meetings, projects, quality assurance activities, and other duties as needed. Salary range: $56.63 – $84.19 Hourly
JOB QUALIFICATIONS
Qualifications
Required: * BSN or MSN from an accredited school of nursing (ASN required for current UCLA Health RN staff). * Current, active California Registered Nurse License. * BLS certificate from American Heart Association or American Red Cross. * Demonstrated skill in delivery of patient care, and outpatient and primary nursing skills. * Expertise in Care Coordination of high risk patients. * Ability to perform historical, physical and behavioral status assessments. * Ability to work as a multi-disciplinary team. * Bilingual language skills in Spanish and English (oral).
