Duke Primary Care Duke Primary Care is a broad network of community-based clinics that offer family medicine, internal medicine, pediatric, and urgent care services. With 400 providers in nearly 50 locations, Duke Primary Care is the largest primary care network in the greater Triangle. Our network saw more than 750,000 patient encounters in fiscal year 2021.

Our Duke Primary Care clinics provide personalized care for adults and children, from sick visits to preventive care, health screenings to immunizations, and annual exams to chronic disease management. Select primary care clinics also provide behavioral health, diabetes education, pharmacy, and other population health services. Duke Primary Care clinics are accredited and designated a Primary Care Medical Home by The Joint Commission.

Our Duke Urgent Care centers offer evaluation and treatment for minor illnesses and injuries 12 hours a day, seven days a week when a patient’s primary care provider is not available. Services also include certain physical exams and vaccinations. A pediatric specialist is available at select locations. Patients can walk in to one of our nine urgent care centers located throughout the greater Triangle or can schedule a virtual visit to receive care from the comfort of home.

Duke Primary Care’s Diabetes Education and Nutrition Counseling Program is an education program recognized by the American Diabetes Association and provides services to individuals referred from within Duke Primary Care as well as outside practices throughout the surrounding counties. The mission of the Duke Primary Care Diabetes Self- Management Education Support Services is to provide current evidence-based education in an open and conducive environment designed to empower participants to make effective lifestyle changes for diabetes self-management and improved quality of life. The services is designed to be an ongoing process to assist a person in implementing and sustaining the behaviors needed for the prevention of complications associated with elevated blood sugars.

This position is embedded at Duke Primary Care (DPC) Knightdale, North Carolina.

Occupational Summary

Coordinate all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of outpatients in a variety of settings. Looking for candidates that are receptive and open to working with a changing, innovative progressive program.

Certified Diabetes Care & Education Specialist (CDCES) Preferred

Work Performed

Level I

  • Provide diabetes self-management education and support along with medical nutrition therapy integrated with the patient's medical goals.  
  • Evaluate, interpret, monitor and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines.
  • Identify and provide age-specific nutrition counseling to meet the cultural needs of the patients and their families.
  • Develop, review, update and implement educational materials to meet the needs of patients and professionals.
  • Function as an integral member of the clinical interdisciplinary team.
  • Maintain registered dietitian (RD) status including continuing professional education per the Professional Development Portfolio.
  • Precept newly hired dietician staff.
  • Comply with team meetings, safety, and attendance standards.

Level II  

  • Performs all the duties in support of and in compliance with performance improvement plans, JCAHO, and other licensing, accrediting, and regulatory agencies.
  • Function in a QI Coordinator role and monitors and facilitates maintenance of DSMES team members’ qualifications.
  • Oversees the planning, implementation and evaluation of the DSMES service.
  • Arranges and coordinates the activities of the Advisory Group.
  • Responsible for maintaining ADA recognition status and participation in the evaluation of the DSMES service’s effectiveness.
  • Assist the department director in reviewing and updating department policies and procedures
  • Plan and coordinate the work schedule and daily work load of the clinical dietitians
  • Other duties assigned by the department director

Preferred Qualifications

Experience

Level I: Prefer individuals familiar with ADA or AADE’s National Standards for Diabetes Self-Management Education and Support. Experience with alternate educational delivery models beneficial.

Level II: Prefer individuals with experience with ADA or AADE’s National Standards for Diabetes Self-Management Education and Support.

Degrees, Licensure, and/or Certification

Registered dietitian (RD) by the Commission on Dietetic Registration

Licensed dietitian/nutritionist (LDN) by the North Carolina Board of Dietetics and Nutrition.

Certified Diabetes Care and Education Specialist (CDCES) by the National Certification Board for Diabetes Educators is PREFERRED.

Required Qualifications at this Level

Education: Baccalaureate degree in Food and Nutrition, Dietetics, Nutritional Sciences, Human Nutrition, or comparable major. Successful completion of coordinated program or dietetic internship of not less than 900 hours under the supervision of a Registered Dietitian.

Level II Same as Level I

Experience

Level I

One year in an outpatient environment preferred. Strongly prefer candidates with experience in motivational interviewing and teaching group classes.

Level II

Two years in an outpatient/education environment with one year supervisory experience preferred.

Level I

Registered dietitian (RD) by the Commission on Dietetic Registration; candidates eligible for registration may be hired with the understanding that registration will be achieved within 90 days of hire.

Licensed dietitian/nutritionist (LDN) by the North Carolina Board of Dietetics and Nutrition; proviso for license-eligible dietitians as above

Level II

Registered dietitian (RD) by the Commission on Dietetic Registration. Licensed dietitian/nutritionist (LDN) by the North Carolina Board of Dietetics and Nutrition
