Position Summary

The Clinical Coordinator Nurse Practitioner or Physician Assistant will work in the Center for Liver Disease & Transplantation within the Division of Abdominal Organ Transplantation in the Department of Surgery.  The position will work under the direction of the Medical and Surgical Directors of CLDT, in collaboration with attending surgical and medical staff, data managers, out-patient nurse practitioners, physician assistants, social workers and financial coordinators.  Position will see patients at 1 or more outreach sites including Flushing Queens. Bi-lingual communication, written and verbal in both Mandarin and English is essential.


Responsibilities

  • Coordinate overall medical and surgical management of patients being evaluated and treated by the liver program, including those with complex liver disease and organ transplantation.
  • Conduct new evaluations and consultations.
  • Provide education about liver disease, transplantation and pre/post hepatobiliary surgery for patients and medical center staff.
  • Management of immunosuppressant and regulation of medications in response to laboratory results.
  • Order and interpret diagnostic studies including but not limited to laboratory and imaging studies, diagnostic impression, and/or form a differential based on the subjective and objective data obtained.
  • Telephone medical advice, interaction with both patients and primary physicians.
  • Formulate a goal-directed plan that includes health maintenance, anticipatory guidance, and appropriate medical and nursing interventions according to the established protocols. 
  • Participate in patient care, education meetings and inpatient rounds as requested.
  • Assist in prior authorizations for medications and radiology testing. 
  • Engages in and supports research activities.
  • Communicates effectively and professionally with patients, significant others, and members of the health care team about the patient's plan of care.  With doctors inside and outside of the CUIMC/NYP Network.
  • Other duties as assigned.  

Minimum Qualifications

  • Nurse Practitioners: Master of Science in Nursing
  • Physician Assistants: Graduate of an ARC-PA accredited, or New York State Education Department approved PA education program.
  • NP - 3 years nursing or related experience.
  • PA - 5 years of related experience
  • Current NYS RN and NP Licenses and ANCC Board Certification.
  • Current NYS PA License and NCCPA Board Certification.
  • BLS Certification
  • DEA Certification

Other Requirements

  • Proficiency with technology and social media platforms.
  • Effective interpersonal and communication skills required.
  • Bi-lingual ??? fluency in Mandarin and English written and spoken.
  • Intermediate to advanced level MS Office skills, experience with EMR.
