Summary

To provide comprehensive professional nursing support in a multidisciplinary cardiac surgery clinic, supporting the Ventricular Assist Device (VAD) patient population.  Coordinate care and follow-up for patients with complex cardiac disorders across the care continuum.  Provide expert nurse instruction and consultation to members of the health care team.  Identify gaps and needs of the patient population and seek to implement interventions to improve patient outcomes.  Develop partnerships with other clinical programs to standardize patient care processes.  Facilitate and maintain quality improvement / LEAN activities

Responsibilities*

Provide expert nursing care to the VAD patient population through individual assessment, evaluation and monitoring of treatment in multiple units.

Coordinate and assess the nursing care given to the VAD patient population in multiple hospital or clinical units.

Design, monitor and evaluate nursing care programs and individualized patient care plans.

Provide liaison and consultation in the area of clinical expertise to members of the healthcare team in multiple units, services and disciplines.

Participate in daily care rounds with multidisciplinary staff.

Document care and patient progress through detailed patient records.

Initiate and participate in the design and maintenance of clinical area quality assurance programs and activities.
Supports standardization and development of departmental protocols, policies, and procedures for Ventricular Assist Device (VAD) patient care delivery.
Provide professional presentations and training to peers, multidisciplinary staff, and the community as they relate to the care and education of the adult VAD patient.  

Provide ongoing VAD patient support per clinical and QI/QA protocols.

Demonstrate knowledge of current medical, surgical and pharmaceutical treatment modalities of the post-cardiac surgery and CHF patient.
Coordinate and provide care management for readmissions, outpatient clinic visits and discharges for VAD patients.

Provide 24 hour on call coverage for the VAD patient population, UMHS healthcare providers and community for all VAD related inquiries.   Call coverage will be shared with other personnel.

Provide education to the VAD patients, families, community, public services; including police, fire, hospitals and other specialists.

Design, develop and maintain patient education programs.

Assist in the development/revision of patient educational guidelines and materials for each circulatory support technology.

Provide expert ongoing telemanagement care to patients, including device management.

Plan and organize travel arrangements and medical contracts necessary for VAD patients long distance travel.

In coordination with Equipment Specialist, coordinates distribution and management of device equipment and ongoing device supply and maintenance.

Participate in ongoing research studies in compliance with federal and organizational regulatory guidelines.

Supervision Received:

Supervision of professional nursing practice is received from the VAD Program Manager.  Functional supervision is received from the designated physicians and advanced practice team.

Supervision Exercised:

Functional supervision may be exercised over medical assistants and other support staff.

• Evening Shift Differential-$3.00 / hour
• Night Shift Differential- $4.00 / hour

•  Night Shift Weekend Differential- $6.90 / hour
• Charge Nurse Differential- $1.00 / hour

Required Qualifications*

Current licensure as a Registered Nurse in the State of Michigan.

Education requirements: Bachelor's Degree in Nursing OR an Associates Degree/Diploma in Nursing with a Masters Degree in Nursing

At least 3-5 years recent experience caring for VAD patient population with demonstrated expert clinical leadership abilities.

Demonstrated excellent critical thinking and problem solving skills.

Demonstrated ability to work collaboratively with and positively contribute to a team approach to patient care, willingness to flex and cover colleagues.

Previous experience with quality improvement / LEAN projects.

Demonstrated high standards of personal and professional conduct and attendance as evidenced by interviews, references, and performance evaluations.

Demonstrated high level of self-motivation to learn and stay current in the treatment of complex cardiovascular disease.

Must demonstrate high level of self-direction and flexibility.

Must demonstrate expert communication, organization and prioritization skills.

Desired Qualifications*

Demonstrated experience with and knowledge of current medical, surgical, and pharmaceutical treatment modalities of advanced heart failure and mechanical circulatory support.

Previous experience in the development of evidence based clinical protocols.

Previous experience in professional presentations and group education.

Demonstrated knowledge and experience in planning, developing, and implementing complex Continuous Quality Improvement activities.

Previous participation in clinical research.

Experience in program development, research and regulatory readiness program planning.
 

Work Schedule

Hours:  32
Shift:  Day shift, Monday-Friday, with evening/weekend on-call rotation.
