The University of Miami, UMH  has an exciting opportunity for a Case Manager/Discharge Planner - Case Management department at the UTower location. This position functions as Discharge Planner for the Care Coordination Department under the direction of the Director of Care Coordination. Discharge Planner will coordinate all discharge needs for the hospital discharges. The Discharge Planner will process the physician order based on the patient’s choice and based on insurance contract agreements with certified agencies. Coordination of all Home Health, Durable Medical Equipment, Orthothetics and Infusion. Utilizes electronic referral system to send referrals for all post discharge needs to licensed providers, assures follow-up with companies for the coordination of benefits and services, obtain any required forms for completion with the physician attending or ordering physician for completion. Assist the social worker with obtaining patient choice when warranted. Communicates the finalization, delivery and acceptance of case to the nursing and care coordination staff.

CORE REQUIREMENTS

A minimum of one year in Hospital Case Management/nursing; a good working knowledge of durable medical equipment, home health and infusion and medical terminology. High School Diploma/2 Year College Diploma Preferred. Must have current BLS certification. Any combination of relevant education, work experience, and / or certifications will be considered.
