Scope of Position/Position Summary:

Case Management is a team process that includes assessment, planning, facilitation and evaluation of the services required to meet the patient’s needs through the healthcare system. The Program Coordinator will provide programmatic support services for the case Management & Social Work areas covering patient’s in UH, Ross, Brain and Spine, Dodd Hall, and East Hospital. The Program Coordinator will be responsible for coordinating CM/SW program support inclusive of throughput/capacity management, discharge planning, and CM/SW extender functions at the direction of the Case Manager and/or Social Worker for selected teams.

Minimum Qualifications :

For Hire –

Bachelor’s degree required

Experience in Acute Care Hospital Setting, experience in assisting with patient care operations preferred

Proficient in Microsoft Office (Word, Excel, Power Point, Outlook)

Experience with EPIC and teletracking systems preferred

Customer service skills
