Department: Care Management
Salary Range: $14.80 to $23.32 per hour based on experience.
Detailed Description:

Communicate with myriad of different payers. Maintain relations with post-acute community.

Timely and expeditious work in a variety of computer programs. Coordinate the service implementation and delivery of post-acute services to various patient populations.

Build, send, and monitor referrals to post-acute care facility and agencies.

Function as a liaison between post-acute providers, case managers, and utilization management analysts. Collaborate with departments and entities to ensure delivery of quality services.

Collaborate and problem solve with care coordination staff to resolve operational problems and promote communication.

Identify and evaluate new or improved processes and make recommendations for improvements.

Other duties as assigned.

Qualifications
Required Qualifications:

Licensed Practical Nurse, Emergency Medical Technician is required or
-
Two years of related medical experience or any college education may also be applied in lieu of above requirement.
