Location: Columbia, Missouri 
Shift:  Evenings, variable days 
Department: Care Management 
Salary Range: $14.80 to $23.32 per hour based on experience. 


Detailed Description: 


  • Communicate with myriad of different payers. Maintain relations with post-acute community. 

  • Timely and expeditious work in a variety of computer programs. Coordinate the service implementation and delivery of post-acute services to various patient populations. 

  • Build, send, and monitor referrals to post-acute care facility and agencies.    

  • Function as a liaison between post-acute providers, case managers, and utilization management analysts. Collaborate with departments and entities to ensure delivery of quality services. 

  • Collaborate and problem solve with care coordination staff to resolve operational problems and promote communication. 

  • Identify and evaluate new or improved processes and make recommendations for improvements. 

  • Other duties as assigned.

Qualifications

 

Required Qualifications: 


  •  Licensed Practical Nurse, Emergency Medical Technician is required or  

  • Two years of related medical experience or any college education may also be applied in lieu of above requirement.
