The GW Cancer Center is seeking a dynamic Cancer Center Associate, Community Health, more commonly referred to as a Community Health Worker, to join a research team examining the impact of community health worker support on cancer survivors’ quality of life.
This position will utilize skills and experience to build trusting relationships with patients to address social risk factors such as food, housing, transportation, and connecting them to resources in the community. The Cancer Center Associate, Community Health will play a pivotal role in implementing the BEAT-C research study, which is focused on reducing inequities in minority cancer survivors. Study components include collecting data on patient experience, social needs, and quality of life.
The Cancer Center Associate, Community Health role requires knowledge (ability to learn) basic clinical concepts and the ability to work with clinical and non-clinical staff to achieve best possible health outcomes for cancer patients.
Job Duties:
  • Administers social risk factor screening and a patient experience survey to patients completing treatment.
  • Manages caseload of patients who may require ongoing follow up such as clinical service coordination, health education, dietary needs, physical or occupational therapy or behavioral changes to improve health and wellbeing.
  • Conducts visits, either virtually, in home, or in a local neighborhood location within the established scope of duties.
  • Partners with community-based organizations to provide patients with direct referrals to community resources.
  • Attends professional development webinars and trainings as requested and applies learning on the job.
  • Collects, tracks and reports important study participant health-related information.
  • Ensures compliance with governmental and accreditation regulations and maintains patient confidentiality.

Performs other related duties as assigned. The omission of specific duties does not preclude the supervisor from assigning duties that are logically related to the position.
Minimum Qualifications:

Qualified candidates will hold a BA/BS in a related discipline. Degree must be conferred by the start date of the position.

Additional Required Licenses/Certifications/Posting Specific Minimum Qualifications:
  • Candidates must have a valid driver’s license and be comfortable with use of their personal vehicle.
  • Candidates may be required to complete Community Health Worker training in the first 6 months of employment and certification following 12 months of employment.
Preferred Qualifications:
  • Demonstrated passion for working within the community you are a part of, or have been a part of in the past.
  • Experience with patient navigation, community outreach, care coordination, or other direct service provision.
  • Knowledge of community-based program services, public benefits, and other resources, as well as how to identify, collate and use new resources.
  • Keen understanding of the role culture plays in a person’s health, including behaviors, language, customs, beliefs, and perspectives.
  • Strength in authentically connecting with people from all walks of life with empathy and humility.
  • Ability to work well within a team, but also an independent self-starter.
  • Adaptability to workflow adjustments and refinements.
  • Excellent organizational skills given coordination of multidisciplinary teams, various logistical components, and data acquisition.
  • Ability to communicate effectively, both verbally and in writing.
  • Adept at problem solving and exercising flexibility and agility.
  • Strong attention to detail and prioritization.
  • Familiarity with Microsoft applications and ability to quickly learn other software such as electronic medical record systems
  • Strong time management and planning skills.
  • Ability to make connections across internal and external teams of care.
  • Willingness to learn and further develop skills reflective of best practices and trends in the field.
  • Spanish language skills strongly preferred.
Typical Hiring Range Commensurate with Experience.
