Job Summary:
Under minimal supervision, responsible for assembling and maintaining patients health information in medical records and charts.
Job Duties:
- Responsible for assembling and maintaining patients' health information in medical records and charts.
- Reviews all forms are to ensure proper completion and validates signatures; completes data entry and verification in a timely manner.
- Communicates with physicians or others to clarify diagnoses or get additional information.
- Answers incoming calls; researches and responds to questions from insurance companies, attorney offices, patients and other health care facilities.
- Processes stat requests for Release of Information based on established guidelines.
- Performs all other duties as assigned.
Education:
High school diploma or GED is required.
Experience:
Five (5) years of experience in medical records or relevant clerical duties is required.
