Ambulatory Clinical Pharmacist- Stem Cell Transplant and Cellular Therapy

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. This position will specifically work with patients before, during, and after stem cell transplants and cellular therapy. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

  • The oncology pharmacy team at UVA is seeking an individual interested in working with patients before, during, and after stem cell transplants and/or cellular therapy. Some of the position highlights include, but are not limited to, the following:

  • Participation in multidisciplinary clinic setting at UVA Cancer Center
    • Multidisciplinary team includes attending oncologist(s), oncology fellows, advanced practice providers, hematology/oncology pharmacist, and numerous medical and pharmacy learners
  • Clinic pharmacist ownership of Beacon treatment plans for respective disease states
    • Cytotoxic chemotherapy
    • Combination therapies including radiation/chemotherapy & chemoimmunotherapy
    • Immunotherapy (i.e., checkpoint inhibitors)
    • Opportunities to design and implement supportive care plans and treatment plans in Beacon for new therapies (newly onboarded medications or literature updates)
  • Patient/caregiver medication education:
    • New start chemotherapy and other anti-cancer therapy counseling (often in conjunction with the hematology/oncology fellow or attending)
    • Oral chemotherapy management
    • Laboratory monitoring
    • Immunotherapy toxicity assessment and monitoring
    • Supportive care optimization (i.e., infection prophylaxis, anti-emetic regimens, radiation toxicities)
  • Opportunities for provider and nursing education on oncology-specific pharmacology and therapeutics
  • Ample chances to mentor and precept PGY2 oncology pharmacy residents
  • Potential teaching roles for VCU School of Pharmacy Oncology Therapeutic modules and/or Oncology Elective course
  • Opportunities to serve on committees within Pharmacy department as well as across the health system enterprise
  • Opportunities to be involved in interdisciplinary quality improvement initiatives and academic research
  • Individual may occasionally rotate into the UVA Cancer Center Outpatient Infusion Pharmacy
Position Compensation Range: $52.46 - $83.94 Hourly

MINIMUM REQUIREMENTS

Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.  
Experience: PGY-1 residency required or equivalent clinical experience (3 years). PGY-2 residency is preferred for specialty areas.
License/Certification: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged. ACLS certification preferred.
 

PHYSICAL DEMANDS

Job requires sitting for prolonged periods; Repetitive motion: typing.  Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs.  May be exposed to fumes, chemicals, blood/body fluids and infectious disease.
