About the Department
 

Through a collaborative practice agreement providing specialized clinical assessment of chronic kidney and end stage renal disease, evaluation, tests, treatments, therapies, medical management and other direct care to patients in outpatient clinics under the care of the University of Chicago Medical Center (UCMC). On-call possibilities may be required. The Advanced Practice Provider may also work with Medical Directors in the DaVita owned Chronic Dialysis Units.


Job Summary
 

Perform duties as an Advanced Practice Provider, providing comprehensive care to patients in the inpatient clinics and outpatient units within the Section of Nephrology in the Department of Medicine.

Responsibilities

  • Provide direct care to chronic hemodialysis and chronic kidney disease patients in both inpatient and outpatient settings, in collaboration with consulting physician and the multidisciplinary health team, to include identification, management, and/or referral of health problems. 
  • Perform the assessment, diagnosis, initiation of delegated medical treatments, independent nursing interventions and evaluation of care.
  • Provide support and follow up to chronic kidney disease patients in the inpatient and outpatient setting.
  • Perform telephone triage regarding patient concerns and complaints, with appropriate communication.
  • Conduct comprehensive or episodic health assessment of patients, gathering pertinent information in a systematic manner from varied sources. 
  • Make referrals to other disciplines as indicated, write weekly progress notes, and assume responsibility for developing diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
  • Analyze all sources of data, establish a medical/nursing diagnosis: initiates and evaluates diagnostic studies, monitors therapeutic procedures and the patient's response to therapy.
  • Conduct comprehensive or episodic health assessments and assist in developing patient care plans within clinical nursing practice and medical care protocols, in collaboration with patient, family members, physician, and other members of the health care team. 
  • Monitor patients’ renal diseases and other comorbid conditions, and incorporates assessment findings into plan of care.
  • Provide anticipatory guidance to patients and their families about the course of therapeutic options available.
  • Order, schedule, interpret, monitor and communicate results of diagnostic tests. Explain necessity, preparation, nature, and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. 
  • Counsel patient concerning lifestyle modifications, medication regiments, side effects and interactions using an appropriate psychosocial communication level.
  • Recognize an emergency situation and initiates appropriate emergency care.
  • Identify complications requiring collaboration and consultation, initiating appropriate action.
  • Promote, has knowledge of, and utilizes appropriate patient care protocols that relate to the physiological and psychological needs of chronic kidney disease patients.
  • Establish appropriate priorities of care.
  • Participate in chronic kidney disease health education in the community. 
  • Participate in basic science and clinical research projects conducted by supervising physician.
  • Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
  • Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
  • Performs other related work as needed.


Minimum Qualifications
 

Education:

Minimum requirements include a college or university degree from an accredited school/college.

Work Experience:

Minimum requirements include knowledge and skills developed through a combination of education and a minimum of 2 years of relevant work experience in a related job discipline.

Certifications:

Current and/or eligible for State of Illinois licensure/registration required. Eligibility to be privileged and credentialed through the Medical Staff Office (MSO). - State of Illinois

  • Ability to conduct physical examinations and health assessments.

  • Ability to interview and obtain medical information.

  • Willingness to enter into written collaborative agreement with a collaborating physician upon hire.
