The Section of Nephrology at The University of Chicago is comprised of nineteen faculty members, two Ph.D. scientists, ten postdoctoral fellows as well as a staff of over 20 research, administrative and clinical personnel that are devoted to a mission of excellence in patient care, education and research. The nephrology program is ranked #23 by US News and World Report survey of best hospitals in the United States.


Job Summary
 

The Advanced Practice Provider will perform duties in the University of Chicago Medicine (UCM) inpatient clinics and with the Medical Director of the Woodlawn DaVita owned Chronic Dialysis Unit. The Woodlawn DaVita Dialysis is a 32 chair off-site unit running multiple shifts through a collaborative practice agreement providing specialized clinical assessment of End Stage Renal Disease, evaluation, tests, treatments, therapies, medical management and other direct care to patients under the care of UCM. On-call possibilities may be required.

Responsibilities

  • Performs duties as an Advanced Practice Provider, providing comprehensive care to patients in the inpatient clinics and outpatient units within the Section of Nephrology in the Department of Medicine.

  • Provides direct care to chronic hemodialysis and chronic kidney disease patients in both inpatient and outpatient settings, in collaboration with consulting physician and the multidisciplinary health team, to include identification, management, and/or referral of health problems. 

  • Performs the assessment, diagnosis, initiation of delegated medical treatments, independent nursing interventions and evaluation of care.

  • Provides support and follow up to chronic kidney disease patients in the inpatient and outpatient setting.

  • Performs telephone triage regarding patient concerns and complaints, with appropriate communication.

  • Conducts comprehensive or episodic health assessment of patients, gathering pertinent information in a systematic manner from varied sources. 

  • Makes referrals to other disciplines as indicated, writes weekly progress notes, and assumes responsibility for developing diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.

  • Analyzes all sources of data, establish a medical/nursing diagnosis: initiates and evaluates diagnostic studies, monitors therapeutic procedures and the patient's response to therapy.

  • Conducts comprehensive or episodic health assessments and assists in developing patient care plans within clinical nursing practice and medical care protocols, in collaboration with patient, family members, physician, and other members of the health care team. 

  • Monitors patients’ renal diseases and other comorbid conditions, and incorporates assessment findings into plan of care.

  • Provides anticipatory guidance to patients and their families about the course of therapeutic options available.

  • Orders, schedules, interprets, monitors and communicates results of diagnostic tests. Explains necessity, preparation, nature, and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. 

  • Counsels patient concerning lifestyle modifications, medication regiments, side effects and interactions using an appropriate psychosocial communication level.

  • Recognizes an emergency situation and initiates appropriate emergency care.

  • Identifies complications requiring collaboration and consultation, initiating appropriate action.

  • Promotes, has knowledge of, and utilizes appropriate patient care protocols that relate to the physiological and psychological needs of chronic kidney disease patients.

  • Establishes appropriate priorities of care.

  • Participates in chronic kidney disease health education in the community. 

  • Participates in basic science and clinical research projects conducted by supervising physician.

  • Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.

  • Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.

  • Performs other related work as needed.


Minimum Qualifications
 

Education:

Minimum requirements include a college or university degree from an accredited school/college.

Work Experience:

Minimum requirements include knowledge and skills developed through a combination of education and a minimum of 2 years of relevant work experience in a related job discipline.

Certifications:

Current and/or eligible for State of Illinois licensure/registration required. Eligibility to be privileged and credentialed through the Medical Staff Office (MSO). - State of Illinois

  • Demonstrated clinical competency, effective leadership and teaching skills.

  • Ability to conduct physical examinations and health assessments.

  • Ability to interview and obtain medical information.

  • Willingness to enter into written collaborative agreement with a collaborating physician upon hire.

  • Once licensed and as needed, willingness to apply for a mid-level practitioner controlled substance license and DEA number.

  • Willingness to be privileged and credentialed through UCM Medical Staff Office prior to start date.

  • Strong interpersonal skills and the ability to work independently.

  • Strong organizational skills.

  • Demonstrated written and verbal communication skills and strong analytical skills.

  • Proficiency in the use of computers.
