A total of one (1), 1.0 FTE Advanced Practice Professional (either NP or PA) will be hired to fill this job opening.

Basic Function and Responsibility

Provide medical care and treatment under the direction and responsibility of a physician.  The selected APP will be in a rotation providing care in three different settings: ambulatory clinic, holding independent clinic; operating room performing the role of first assist in Urologic surgeries (including robotics), and on the inpatient floor providing care for patient rounding, discharge, and first responder for daytime consults. Clinical subspecialties include General Urology (including general pediatric urology), Endourology, Female Urology, and Urologic Oncology.   Primary clinic site: West Shore Urology, Muskegon, Michigan.  Hospital and surgery center locations include Muskegon Surgery Center, Trinity Health Muskegon Hospital, and North Ottawa Community Hospital.

Duties and Responsibilities

  • Under authority of a designated supervising physician, effectively evaluate and manage the care of Urology patients
  • See new and follow-up patients and provide intervention that is problem oriented for their urological conditions
  • Obtain a relevant health and medical history 
  •  Perform a physical examination based on age and history 
  • Conduct preventive screening procedures based on age and history 
  •  Identify actual and potential health problems and needs 
  • Update and record changes in health status
  • Formulate the appropriate diagnoses based on the history, physical examination, and clinical findings 
  • Identify needs of the individual, family, or community as a result of the evaluation of the collected data
  • Establish priorities and a mutually acceptable plan of care to maximize the health potential of the individual, family, or community. 
  • Order appropriate diagnostic tests and laboratory tests
  • Identify appropriate pharmacologic agents 
  • Identify non-pharmacologic interventions 
  • Develop a client/family education plan
  • Update daily patient list.
  • Daily patient rounding 
  • Provide patient education on PCT flushes, foley management, SPT, etc.
  • Management, ostomy care and catheter care including intermittent self-catheterization.
  • Prepare patients for timely discharge process. Identify needs of the individual, family, or community as a result of the evaluation of the collected data. Report problems to the attending or on call physician.
  • Evaluate acute postoperative patients. Report findings to attending or on call physician. 
  • Discuss inpatient management with consulting services. 
  • Assist in minor procedures as directed by attending physician.
  • Advise patients regarding their urologic condition, wound care, and pre and post operative care.
  • Base interventions upon scientific principles, theoretical knowledge, and clinical expertise; individual to the specific situation; and consistent with teaching and learning opportunities. The interventions may include: 
  • conducting and interpreting diagnostic tests 
  • prescribing pharmacologic agents 
  • prescribing non-pharmacologic therapies 
  • providing relevant patient education 
  • making appropriate referrals to other health professionals and community agencies
  • Provide education to patients regarding their urologic care including catheter care and intermittent self-catheterization
  • Document care according to Michigan Medicine standards
  • Update patient problem summary list including, medications, diagnoses, allergies, procedures, past medical history, family history, and social history
  • Follow up on ordered diagnostic tests and labs
  • Perform History & Physical Pre-operative Assessments to include: 
  • Brief physical examination 
  • Obtaining surgical consent
  • Detailed review of patient medications
  • Providing pre-surgical teaching/instruction 
  • Addressing patients? questions and concerns
  • Ordering additional labs and diagnostic testing as needed 
  • Present patients, when appropriate, to the supervising physician and assist in care, treatment, and follow-up as needed
  • Perform and/or assist in office procedures to include post-void residuals, post-operative voiding trials, and other necessary procedures
  • Provide phone consultation/triage for patient concerns and prescription refills.
  • Work independently to see new and return visits under supervision of faculty
  • Work collaboratively with supervising physicians, other midlevel providers, and clinic personnel to successfully deliver high quality care to our Urology patient population
  • Assist in the evaluation and improvement of processes and supplies
  • Analyze, compare, and evaluate various courses of action and have the responsibility to make independent decisions free from immediate direction within the scope of his/her assignments
  • Maintain flexibility while providing efficient and effective support to providers and patients
  • Act as first assistant in operating room cases.
  • Manage urologic consults in an inpatient setting under physician supervision
  • Managing postoperative care of inpatient surgeries, including robotic radical prostatectomies and urologic reconstructive surgeries under physician supervision
  • Potentially performing or assisting in inpatient bedside procedures including catheter placement, cystoscopy and incision and drainage under physician supervision
  • Other duties as assigned 
     

Supervision Received

  • Administrative and functional supervision received from West Shore Urology Ambulatory Care Unit Medical Director
  • Administrative time keeping would be performed by Ambulatory Administrative Manager  
     

Supervision Exercised

  • None

NP Requirements

  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Current specialty certification to practice as an Adult Gerontology Acute Care Nurse Practitioner. 
  • Graduate of an accredited Nurse Practitioner program.
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN) or American Nurses Credentialing Center (ANCC).  
  • Current DEA registration required
  • AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center
  • ACLS

PA Requirements

  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant  (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Current license to practice as a Physician Assistant in the State of Michigan.
  • State of Michigan Controlled Substance License Required  
  • Current DEA registration required
  • AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center
  • ACLS

  • At least one (1) year of working full-time experience in an Advanced Practice role in Urology.
  • Specific experience as an Advanced Practice Provider caring for surgical patients in an ambulatory care setting.
  • Experience with surgical patient care. 
  • Excellent interpersonal and patient service skills. Ability to work with minimal supervision, and as part of a collaborative team. 
  • Experience working in EPIC.
  • Experience working within a model of independent patient visits.

Employment is contingent upon successful completion of the credentialing and privileging process.


Hours:  40 hours Day Shift
 


This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
