Job Summary

 

The Family or Adult-Gerontology Nurse Practitioner will provide primary health care to clinic population. Primary health care is defined as (1) the care the consumer receives at the point of contact with the health care system; and (2) the continued care of the individual as a health care consumer.  The care is two-dimensional: (1) identification, management and/or referral of the health problems; and (2) the maintenance of the consumer's health by means of preventive and promotive health care action.

 

***While this position will be primarily located in the Delphi clinic, occasional travel to clinics in Wolcott, Monon, and Burlington may be required as FNP coverage needs demand.***


Required:

  • Master of Science degree in Nursing with specialization as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner 
  • A minimum of 1 year experience as a Registered Nurse required along with advanced degree as a Nurse Practitioner 
  • Indiana State RN and APN licensure; certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner by National certifying body. 
  • Certified to complete Commercial Driver’s License Examinations.

 

Preferred:

  • Additional years of nursing experience
