Position Summary

This position is responsible for supporting the Connections centralized intake and referral system. The Community Navigator will meet with families desiring assistance and use interviewing techniques and formal assessments to determine the program which can most effectively meet the families’ needs. The Community Navigator will also assist the family in accessing community resources that are needed immediately. The Community Navigator for this program will serve as the point person for the Department for Children and Families for all ABC Home Visiting program referrals for all three counties in Northeast Kansas (Wyandotte, Douglas, and Leavenworth). The Community Navigator will screen and accept referrals and provide the DCF referring worker with the ABC Home Visitor contact. Reports to the Assistant Director of Health and Screening Services.

Required Qualifications

Bachelor’s degree in Social Work, Nursing, Psychology, Special Education, Early Childhood Education or a related field.
Experience providing case management services.
Excellent rapport-building and interpersonal skills.
Attention to detail and good follow through.
Ability to identify and respond to the social, economic and cultural experiences of participants.
Valid driver’s license and access to personal car required.

Preferred Qualifications

Bilingual English/Spanish.
Knowledge of resources in Wyandotte, Douglas, or Leavenworth counties.
Experience administering child development and/or family support screening and assessment instruments.

Meet with families as soon as possible after the initial referral is made from the Department for Children and Families. Interview family members to determine needs. Utilize screening and assessment tools as appropriate (child development, maternal depression, etc.). Prioritize the families’ needs in partnership with the family.
Explain the services of the ABC Home Visiting program and other resources for the family based on their initial screening results.

Answer the Connections phone and respond to inquiries about Connections and/or Project Eagle enrollment process as needed.
Refer families to other community resources (WIC, emergency assistance, etc.) as necessary to meet the families’ immediate needs.
Ensure the appropriateness of the placement by following up with the family to determine satisfaction with the program services.
Enter family information into the management information system.
Respond to questions/concerns from families and partnering agencies regarding the participation process for Connections.

Participate in monthly staff meetings and ongoing professional development.
Participate in staff development activities and demonstrate increased knowledge, understanding and ability to transfer knowledge to practice with families.
