Job Description:

  • Provides individual contact with patients to identify and develop client directed
    treatment plan goals and monitor ongoing achievement of goals.
  • Supports patients in navigating the clinic system and community resources.

  • Engages patients expected to start ARV regimens in an assessment of readiness
    for treatment, provide education on HIV medications, anticipated benefits/sides
    effects and importance of adherence. Assesses patient needs upon onset of
    medication.

  • Provides individual and group educational skill building opportunities to foster
    adherence to medications, identifies strategies to improve adherence to health
    routines, communicates with providers on additional issues to increase
    engagement in care and adherence to treatment;

  • Enhances engagement in care and adherence by conducting patient reminder
    calls, notification and follow-up per program policy and procedures.

  • Meets with patients during clinical visits to introduce and discuss health promotion
    and disease prevention with available literature. Participates in continuing
    HIV/AIDS education.

  • Provides administrative assistance with clinical operations and medical records as
    needed, including scanning electronic medical records and being available to
    conduct HIV testing in clinic and community.

  • Adheres to all confidentiality policies. It is a direct violation of company’s policy to
    share the names or case facts concerning any client, patient or volunteer of the
    Clinic with any other person with the exception of those actually involved in the
    care of the patient/client. Any release of confidential information to any other entity
    shall be performed by authorized personnel only and shall be accompanied by
    proper written authorization from the patient/client.
Expected Salary:

$16 to $18 per hour depending on education and experience.

Minimum Requirements: High school diploma or equivalent and one year of experience with children or families.
Preferred Qualifications:
  • At least 1-2 years' experience with HIV population preferred, either via work environment or participation in volunteer activities.
  • Basic computer proficiency
