Job Summary
Located in Frankfort, Ky. This position will focus on identifying, planning, and preparing individuals to transfer from ICF/MR facilities to the community as part of the Kentucky Money Follows the Person program. The position will work within the Transition Team, developing, implementing, and evaluating individual plans of care. Assess individuals requesting transition, thoroughly review and understand medical records, communicate effectively with medical staff. Assess need for assistance with locating housing. Will be working in Frankfort with some statewide travel required. This position is funded by a joint federal/state grant from the Cabinet for Health and Family Services.

Skills / Knowledge / Abilities
Clinical/Assessment expertise, written and oral communication, attention to detail, understanding of risk assessment & monitoring for quality improvement, and Microsoft office.
