Description

 

The TCHATT Intake Specialist reports to the TCHATT Sr. Program Manager and is responsible for screening students enrolled in the TCHATT program to determine the appropriate treatment path.  This position requires the ability to utilize screening and assessment tools with appropriate training, to assist in providing measurement-based care, and perform psychosocial intake appointments.  Position requires the ability to function effectively as a team member while also having the ability to work independently with moderate supervision.  This position is part of the Texas Child Health Access Through Telemedicine and is funded by the Texas Child Mental Health Care Consortium.

Major Responsibilities / Duties / Critical Tasks:

·         Work in an inter-professional model and collaborate with pediatric psychiatrists, licensed professional counselors, care coordinators, and administrative staff to identify the psychological needs of students referred for care.

·         Provide assessments via tele format by utilizing evidence-based screening and assessment tools.

·         Collect information concerning patients’ clinical background and referral needs for a psychosocial intake.

·         Ability to manage crisis and utilize crisis interventions when needed.

·         Coordinate with TCHATT professionals, schools, and families to minimize obstacles and increase patient and provider satisfaction.

·         Maintain appropriate, timely, and efficient documentation.

·         Maintain confidentiality of information as appropriate in all aspects of care.

·         Participate in team treatment meetings and other team meetings as needed.

·         Maintaining required documentation within EMR.

·         Demonstrate professional behavior in all interactions and utilizes proper telephone etiquette during every call.

·         Other duties as assigned.

·         Maintaining clinical documentation in an EMR.

·         Knowledge of educational practices supporting mental health and social emotional learning in K-12 settings.

·         Knowledge of managed care, medical terminology and patient referral process.

·         Ability to be flexible, make sound decisions, and demonstrate problem-solving skills.

·         Excellent written and verbal skills.

·         Extensive experience with the use of Microsoft office suite programs

·         Ability to quickly grasp new software.

·         Bilingual (English/Spanish) language proficiency preferred.



Qualifications

 

Required Education / Experience:

·         Bachelor's degree required.

·         Master’s degree in Social Work, Psychology, or related field strongly preferred.

·         Licensed provider (ex: LMSW) or equivalent experience.

·         Experience working with pediatric behavioral and mental health patient populations.

·         Two (2) years of professional experience working in a health care or educational (preferably k-12) setting.

·         Excellent written and verbal communication skills.

·         Demonstrated knowledge working with deadlines and under pressure and the subsequent ability to prioritize tasks.

·         Ability to be flexible, make sound decisions and demonstrate problem-solving skills.
