Posting Summary
Rutgers, The State University of New Jersey is seeking a Senior Care Coordination Specialist NJMS-Medicine-Infectious Disease. The Senior Care Coordination Specialist works as the Licensed Social Work/ Licensed Associate Counselor Mental Health Clinician and contributes to the success of the program by managing the daily workflow to achieve high quality care management delivery and positive clinical outcomes, while meeting the administrative and contractual performance targets, documentation standards and billing requirements of the grant program(s).
Among the key duties of this position are the following:
  • Assess and works collaboratively with the patients, medical provider, designated family/significant others and community support service providers to address the needs identified in the patient’s health and psychosocial risk assessment and implement a plan of care based upon the needs identified
  • Facilitates the development and implementation of a comprehensive, multi-disciplinary plan of care for patients with complex mental health and/or substance abuse and socio-economic problems, in collaboration with the physician and other members of the healthcare team
  • Counsels and educates patients in individual and group sessions to assist them with managing experiences of trauma, illicit substance use, mental and physical illness, poverty, unemployment or abuse.
  • Assists in the coordination and monitoring of the unit’s buprenorphine program. Follows all guidelines and standards regarding record keeping and documentation.
  • Provides co-occurring behavioral health psychotherapeutic interventions to provide psychosocial support with continued adherence to treatment and engagement in ongoing treatment and care or referral to higher level of community outpatient programs such as IOP or Partial Hospitalization

Qualifications


Minimum Education and Experience
  •  Master’s Degree in Social Work, Counseling or a related field with a license to practice in the State of New Jersey plus two (2) years post Masters experience. 
  • Minimum one year managed care and/or case management experience required. 
  •  At least two (2) years of clinical experience in the care of people with HIV/AIDS
Certifications/Licenses
Required Knowledge, Skills, and Abilities
  • Patient experience in the area of mental health and/or substance use required. 
  • Computer literacy with competence in Microsoft Office components.
  • Detail oriented with excellent organizational and communication skills. 
  • Ability to effectively present information and respond to questions from groups of managers, clients, and customers. 
  • Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. 
  • Excellent written and verbal communication skills.  

Preferred Qualifications
  • Bilingual (Spanish) 
  • Excellent organizational skills. Knowledge of ICD10 and DSM-V coding

PHYSICAL DEMANDS:
Position requires standing to 1/3 of the time, walking to 2/3 of the time, requires sitting 1/2 of the time, talk or hear over 2/3 of the time. There are no special vision requirements.

Clinical environment. Moderate noise (examples: business office with computers and printers, light traffic).
