Essential Duties:

  • Demonstrates proficiency and knowledge of current skin, and wound management.
  • Provides care to a busy clinic of 6-9 patients per day.
  • Provides consultation to Home Health Agencies and Care Homes.
  • Provides consulting and direction to the patients, family members, and health care providers and physicians regarding wound care management.
  • Uses critical thinking skills and sound judgment based on evidence based practices when making the decision for appropriate wound type treatment
  • Takes responsibility for personal professional development by keeping up with current research in wound care management and shares the findings with the medical staff and clinical team
  • Reviews all policies and procedures and updates that pertain to wound care and maintains that knowledge base throughout decision making process.
  • Attends Wound care and Hyperbaric lectures, seminars which deal with wound care and informs the hospital of the latest developments in wound management
  • Works effectively with case managers, staff and other departments. Acts as a role model for the staff.
  • Delegate's portions of therapy appropriately to appropriate support staff for overall department efficiency.
  • Ensures staff makes recommendations for next level of care.
  • Ensures staff includes patient/family goals consistently for all patients.
  • Recognizes changes in a patient condition and communicates effectively with the team and physician to adjust the treatment plan.
  • Provides assistance to panel physicians.
  • Confers with others to identify learning needs and suggests or selects most appropriate learning solutions for addressing those needs
  • Maintains department equipment and materials in a condition to promote efficiency.
  • Works with Director to recommend specialty grafts, VAC equipment, or other modalities for patients.
  • Actively participates in the ordering of adjunctive therapies to include Hyperbarics.
  • Demonstrates the ability to independently manage department projects and completes on a timely basis. Demonstrates knowledge of the change process through project implementation.
  • Ensures that department is in compliance with JCAHO requirement.
  • In all aspects of consultation:
    o Initiate care to meet patient's needs.
    o Coordinate plan of care with physician/consultant.
    o Assess patient response to treatment.
    o Evaluate care provided by health care providers.
    o Documentation of teaching instructions and patient's response. .
  • Collaborates with medical director and other disciplines in the coordination, planning and evaluation of the Wound Care Program.
  • Communicates individual and unit issues with the appropriate Unit Coordinator or Director and offers possible solutions and plans.
  • Maintains a professional, open relationship with administrative staff and utilizes them in coordination, planning and implementing projects.
  • Coordinates and provides clinical education of wound, and skin integrity to health care providers and physicians
  • Maintains BLS Instructor certification and assists with BLS recognition and recognition as needed.
  • Keep current with new products or technologies in WCC nursing.
  • Plan & Establishes system (S) for learning events to ensure that they meet the established goals and objectives
  • Scribing (documentation assistance) while in the presence of the clinic physician. The minimum qualifications for documentation assistance are an RN, MA, and RT. The allowable scope of activities that can be performed are to include Procedures, Physician Orders, Medications, Superbill. Yearly assessment of performance and continued competence will be established. All scribes will meet the same competency and training described above.
    Education and Experience
    Required: Minimum Education: Knowledge and education necessary to obtain R.N. license in the state of California. Current American Heart Association BLS certification. Knowledge and education necessary to obtain a nationally recognized Wound Care Certification within 2 years of hire. Knowledge of/or experience with negative pressure wound therapy, HBO, and ostomy care preferred. Minimum Experience: Two or more years of wound care or hyperbaric experience preferred. Prior leadership and/or teaching experience preferred. Spanish or Vietnamese as a second language desirable
    Mandatory Requirements
    Hospital Mandatory Education Requirements: Orientation, Environment of Care, OSHA, Infection control, Abuse/Neglect, Ethics, etc.
