Job Summary:

This position has multiple responsibilities including coordinating the implementation of the Centers of Disease Control and Prevention (CDC) colorectal cancer (CRC) grant. They will assist in implementing evidence-based interventions (EDBI) in collaborating Federally Qualified Health Centers (FQHCs) and support in the process to increase colon cancer prevention, screening and early detection for effective treatment. This position will include community outreach involvement such as presentations to and working with targeted populations.

Major Responsibilities / Duties / Critical Tasks:

·       Manages the day-to-day activities of the project by supporting a cohesive team effort to accomplish approved goals, milestones, and metrics.

·       Identifies and develops affiliations with target groups for the implementation and assessment of education and outreach projects that address issues in colorectal cancer (CRC).

·       Prepares content and materials and delivers CRC educational presentations for diverse audiences.

·       Coordinates logistics for various CRC meetings, programs, conferences and research activities.

·       Maintains ongoing communication with diverse organizations, agencies, and individuals to expand the distribution network for available resources and programs.

·       Designs and updates as needed, materials to promote the work, services and opportunities to improve CRC for broad-based target audiences.

·       Distributes and solicits evaluation of materials and programs designed to promote CRC using delivery methods appropriate to the task.

·       Participates in meetings, conferences and workshops as requested/required.

·       Prepares program activities as requested.

·       Maintains records, images and materials related to program activities as an archive and to assist with the preparation of regular and special request reports.

·       Work with identified experts to develop specialized curriculum and education programs for patients and the community to address trends in chronic disease.

·       Initiates contact and collaborates with local organizations to promote access to healthcare education, screening and treatment (as needed) for colon health.

·       Maintains frequent communication with project and other staff through personal visits, letters, e-mail, phone calls and web-conferencing.

·       Assures that all required reporting is completed accurately and in a timely manner according to established deadlines.

·       Collaborate with the GI clinic and other departments as necessary to ensure a smooth transition throughout the patient’s care.

·       Handle confidential/sensitive information with discretion and adherence to HIPAA rules.

·       Promote the UTHSCT mission, vision, and values by action.

·       Other duties as assigned.

Qualifications

 

Required Education / Experience:

  • Bachelor's Degree in public health, community health, health education or a related field.
  • Two (2) years of health/clinical experience or equivalent health-related education.
  • Experience and education may be substituted for one another.

Preferred Education / Experience:

  • Graduate Degree preferred.
  • Knowledge of community resources and experience working with a variety of community and/or faith-based organizations preferred.
  • Texas DSHS Community Health Worker certification course or Certified Health Education Specialist preferred.
