Position Summary
Under general direction, regularly interacts with patients, physicians and payor partners to impact medication adherence and fulfill other position responsibilities as defined by this role's scope of practice.
Job Responsibilities

  • Advises and consults with patient and families on medication use, dangers, side effects and composition.
  • Documents assessments, observations, recommendations, interventions and service rendered in the Electronic Medical Record system.
  • Interfaces with payors as needed.
  • Applies subject matter expertise in medication adherence strategies, medication reconciliation and transition of care services.
  • Assists in resolving medication dispensing system problems.
  • Assists with reviewing and interpreting medication orders. Verifies orders in accordance with pharmaceutical care standards and prescribed policies and procedures. Reviews patient profiles and history for new orders or order changes.
  • Ensures the proper storage and maintenance of pharmacy inventory. Assures stock is rotated and expiration dates are checked.
  • Ensures patient safety is maintained at all times.
  • Responsible for maintaining a sterile environment. Ensures standard infection control precautions are observed.
  • Provides in-service education and/or orientation concerning drug use to health professionals when necessary or as appropriate.
  • Monitors performance metrics to identify areas for opportunity.
  • May develop, maintain and/or disseminate departmental or functional specific policies and procedures. May also be responsible for maintaining a departmental standard operating procedures (SOP) manual if available.
  • If applicable, assists with preparing medications and dispensing of medication to outpatients, including unit dose dispensing, IV admixture, chemotherapy, TPN, investigational drugs and other sterile products and extemporaneous preparations.
  • If applicable, completes written records of dispensing for all medication including controlled substances and investigational drugs as required by policy and regulation.
Education
  • High school diploma or GED
Experience
Approximately 1-3 years of experience in a general pharmacy practiceKnowledge, Skills and Abilities
  • Additional experience or education in a specialty area of pharmacy practice is preferred.
Licenses and Certifications
  • NYS licensure as a Pharmacist Technician
