Position Summary
The Patient Outreach Specialist will assist with implementation of long-acting antiretroviral medications with a focus on rural areas and provider education. They will outreach of patients with HIV in our service area and help with access to long-acting antiretroviral medications. They will develop and maintain relationships with rural clinics, community partners and speak publicly with community members and at community events about participating in the program. Develop and print digital communications for the ECHO program and share it with clinic staff and participants from rural sites. Assist with scheduling, rescheduling, and maintaining engagement in care and adherence for rural patients enrolled in the program and deliver interventions such as text messaging reminders and patient portal communication. Act as a liaison between patients, the Grand Island clinic, and the UNMC clinic to ensure patients are engaged in care, adhere to injection visits, and connect with providers through telehealth visits. Monitor and update the electronic medical record and the database in REDCap. Monitor enrollment for the program to ensure we are achieving the targeted enrollment and racial and ethnic diversity of participants and work closely with clinic and study staff to evaluate and update recruitment strategies and track enrollment.
