Posting Summary
Rutgers, The State University of New Jersey, is seeking a part time Patient Navigator for the department of Medicine at the Robert Wood Johnson Medical School.
Rutgers, The State University of New Jersey, is seeking a Patient Navigator for RWJMS-Medicine-Hematology/NJ Regional Hemophilia Program. This position provides patients with assistance in access to healthcare, maintenance of comprehensive bleeding/clotting disorder treatment and medical case management services.
Among the key duties of this position are the following:
  • Acts as home care liaison at the NJ Regional Hemophilia Program to ensure timely and coordinated patient access to home care services including medication and nursing as part of the treatment protocol/standard of care for patients with bleeding disorders.  
  • Directly supports access to care by assisting patients to assure attendance at comprehensive annual evaluations, follow up visits, consultations, obtaining referrals, and assisting patients with associated documents (school & travel letters, ie). Collaborates with HTC team in identifying patients in need of this support, contacting patients with appointment reminders, arranging transportation, and assisting non-English speakers in accessing translation services.
  • At the direction of the social work coordinator, assists social work coordinator in providing case management and advocacy for identified bleeding and clotting disorder patients who face barriers to receiving comprehensive medical care including barriers relating to inadequate insurance coverage, lack of transportation and language barriers. 
  • Facilitates access to patient assistance programs for bleeding and clotting disorder patients including provision of program information/forms/applications with direct patient support to ensure submission of paperwork and follow up to confirm services were received. 
  • In conjunction with the social work coordinator, facilitates communication with Spanish speaking patients with psychosocial needs and acts as liaison for Spanish speaking patients requiring additional supports relating to their bleeding disorder treatment and care.
  • Ensures patient access to specialized bleeding and clotting disorder diagnostic testing such as specialized laboratory and genetic testing by contacting insurance plans to gain authorizations. For uninsured patients, assistance with patient applications for Medicaid/Charity Care or other available programs. 
  • In conjunction with the social work coordinator and program manager of the NJ Regional Hemophilia program, assists in the implementation of the HRSA mandated program to increase access to care for the underserved population which includes Latino/a and other Non-White populations (per HRSA). Duties include Direct patient outreach and provision of educational material.

Qualifications


Minimum Education and Experience
  • Bachelor’s Degree in a Health, Public Health, Social Work, or other relevant field required; plus two (2) years’ experience in health or community outreach/advocacy related field. 
  • Equivalent education, experience, and/or training may be substituted for the degree requirements.  
Certifications/Licenses

  • Computer literacy with competence in Microsoft Office components required.  
  • Must possess excellent written and verbal communication skills; strong organizational skills; detail-oriented and ability to manage a multitude of tasks.  
  • Able to function independently; Ability to multi-task; be empathic, compassionate; committed, dedicated, persistent; & resourceful.
  • Fluency in Spanish  

Preferred Qualifications
  • Experience working in case management in a healthcare setting.
  • Experience working with Latino/a community.
