Position Description


Advertised Job Summary
Promotor de Salud is the Spanish term for Community Health Worker and is someone who promotes health, provides resource navigation and outreach in their own community by providing leadership, health education and referrals to care. Promotores (men) and Promotoras (women) understand the same culture and speak the same language as the people they serve.
They are culturally-competent health educators who link people to health and human resources, encourage peers to seek care, empower community members, and promote healthy habits.
Community Health Workers (Promotores) will spread culturally appropriate health messages in their communities and assist those they serve in navigating systems related to health, legal, and other social service resources.
The CHW will be responsible for achieving the PASOs Health Connections program goals assigned for the region.

This is a full-time position and will report to the Midlands Program Coordinator

Required Education and Experience
Bachelor’s degree and 1 or more years of experience, which may be substituted by an equivalent combination of certification, training, education, and/or experience. Valid SC Driver’s License. Must be bilingual/bi-cultural in English and Spanish. Experience working with the Latino community. Ability to work flexible hours to meet the needs of participants, including evening and weekend hours as needed. Must be able to travel.
Preferred Qualifications
Degree in Health education, health promotion, or a closely related field such as psychology, social work biology, sociology, or education.
Knowledge/Skills/Abilities
Ability to use intermediate functions and features of the MS Office Suite, including Outlook, Word, Excel, and PowerPoint. Ability to navigate the internet, including usage of and the ability to successfully and accurately utilize web-based programs and applications. Ability to learn and effectively use electronic medical record software.

Serve as an information source for Latino families referring them to local resources and services, and answering their questions when possible. Answer phone calls and respond to messages/inquiries in a timely and polite manner. Communicate with participants in a courteous, professional, cooperative, and mature manner, both in person and over the telephone. Remain abreast of federal, state, and local community resources to provide interventions that address access to care and social determinants of health for participants and assist them in enrolling in local programs for which they qualify. Assist individuals and families with assessing program affordability, eligibility requirements, education about programs, and enrollment in various benefit programs including but not limited to Medicaid, WIC, and SNAP. Facilitate problem-solving of consumer grievances, complaints, or questions about programs and services

Participate in Semi-annual PASOs Team Professional Development Trainings. Participate in all other program-required trainings and meetings
