Job Summary
Under the direction of a faculty sponsor and in collaboration with the program support team, coordinates the care of patients through the abortion access process. Acts as a clinical liaison for patients, across medical services and clinical departments. Assesses clinical, emotional, psychosocial, financial, and other patient needs. Evaluates and manages patient symptoms before, during and after treatment.
Responsibilities
Provides care coordination & patient education in Complex Family Planning.
Supports discharge planning for the patients in Complex Family Planning.
Supports transitioning of incoming patient referrals to UCMC Complex Family Planning, including OON and self-pay patients.
Facilitates resolution to patient care gaps and monitors follow up/completion by both proactively reaching out to patients and addressing direct phone calls and messages from patients.
Acts as liaison for patients, families and their multidisciplinary clinical team.
Provides lead nursing support in the outpatient setting.
Educates/reinforces education with patients and families on Complex Family Planning services, including complex abortion care.
Clinical documentation review to improve LOS.
Develops and/or coordinates development of patient education materials, clinical pathways, and quality studies.
Develops acuity tools and oversee other QI projects.
Serves as primary point of contact for patients both prior to their appointment and after discharge to address questions and patient care needs to address questions and patient care needs to ensure high quality of care.
Leads comprehensive or episodic health assessments. Solves complex problems in patient care and recommends appropriate treatments.
Develops patient care plans within clinical nursing practice and medical care protocols and prepares patient health summaries. Provides expertise on effects of scheduled diagnostic and therapeutic procedures to the patient.
Evaluates outcomes of care delivered to patients and initiates changes in the nursing approach accordingly.
Develops collaborative practice arrangements.
Collaborates with and provides expertise to nurses, patients, families, physicians, and community resources for coordination of patient care.
Selects appropriate communications approach to achieve desired outcomes of patient care.
Requires specialized depth of knowledge and skills for professional development. Interprets quality assurance issues on the unit, and recommends best practices. Provides expertise to staff on or off the unit for clinical skills.
May be responsible for direct patient care.
Develops and implements treatment plans and provides guidance on therapies. Provides follow-up with patients.
Implements evaluation programs regarding the quality and effectiveness of nursing practice or organizational systems.
Guides clinical practices for patient care or research projects, such as by reviewing patient records, monitoring compliance, and meeting with regulatory authority.
Performs other related work as needed.

Education:
Bachelor’s of Science in Nursing (BSN).
Experience:
Clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
Experience in Abortion Care or Obstetrics and Gynecology.
Licenses and Certifications:
Registered Nurse with a current Illinois license.
Basic Life Support (BLS) Certification - American Heart Association or Other.
Preferred Competencies
Must have general computer skills and be adaptable to changing working situations and work assignments.
Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.
Superior phone communication skills and Ob/Gyn triaging.
