Job Summary

The Division of Geriatrics seeks a Nurse Practitioner to support the UCSF 360 Wellness Center consult service.  UCSF 360 Wellness Center was created in 1984 to offer a range of high-quality services in a single location for people living with HIV/AIDS, and this position requires optimizing the care of older adults in the 360 Clinic when consults are requested by providers. 

The clinical work involves providing consultation services from Geriatrics to the 360 Clinic by completing structured assessments, patient exam, and making recommendations to the clinic and Primary Care Physicians (PCPs) for ongoing care/ home management as indicated.  It involves coordination with pharmacists, 360 clinic providers, geriatricians, social workers, community-based providers, and other interdisciplinary professionals and/or services to create an optimal discharge or ongoing plan of care for the patient. 

The position does not involve direct clinical management of patients in the 360 Clinic.  The committed hours are based on ad hoc needs of the 360 Clinic through scheduled consults, which may be conducted in person or via telemedicine.

Our ideal candidate is an experienced Nurse Practitioner who possesses the advanced education, knowledge, clinical skills, and experience necessary to provide expert advanced nursing care in the area of geriatric specialization.  They will be guided and follow the policies and standardized procedures, rules and regulations, requirement of supervision under CA statute, and bylaws of the UCSF Health, some Nursing, Medical Staff, and any unique policies related to the provision of patient services. In California, nurse practitioners (APRNs) can practice, bill, and provide independent health care services under a standardized procedure in collaboration with a supervising physician.

The Division of Geriatrics’ work is grounded in a deep commitment to and respect for older persons. We are specialists in aging. Our expertise is in optimizing the health of older individuals. We work in the community and on virtually every UCSF campus – Parnassus, San Francisco Veterans Affairs Medical Center, Zuckerberg San Francisco General Hospital, Mission Bay, and UCSF Center for Geriatric Care. Required Qualifications

  • One (1) year NP experience providing care for patients living with HIV/AIDS or working/serving in HIV care communities.
  • Ability to travel to provide in-home care.

Preferred Qualifications

  • Geriatrics experience inpatient or outpatient
  • Experience in precepting NP students
  • Ability to qualify as UCSF Health trusted interpreter in Spanish
  • Experience using EPIC

License/Certification

UCSF Medical Center and Campus requires certain healthcare providers to be credentialed. In most cases this is done through its Office of Medical Affairs & Governance (OMAG) prior to the first working day. Credentialing and approval of the Standardized Procedures takes between 60-90 days but can vary.
