Position Summary
Flexible work arrangements, including virtual/hybrid work locations, are an integral part of the School of Medicine’s Working Forward initiative. As such, this position’s work location is designated as hybrid. Please note that the designated work location is subject to change based on the unit’s business needs.
The UNC Children’s Complex Care team currently serves over 400 children and families from across North Carolina, primarily in the Piedmont Region in the cities and counties around Raleigh, Durham, and Chapel Hill. With this new Nurse Coordinator position, we expect to grow to serve about 500 children and families from around the state. Our team provides clinical care and care coordination services across the spectrum from hospital to community. Our goal is to help families and clinical care teams provide more efficient, effective, and family-centered care that optimizes quality of life and well-being for children with complex needs and their families.
The Complex Care Nurse Coordinator will help manage the UNC Children’s Complex Care Program. S/he will provide direct care management for a panel of eligible CMC longitudinally at inpatient and outpatient points of care and will help to manage the infrastructure of the Complex Care team.
Job scope and responsibilities:
Medical- Patient Care/Nursing
The first co-primary responsibility of the Nurse Coordinator will be to facilitate effective transitions of care between the inpatient and outpatient setting for high-risk children with medical complexity (CMC) enrolled in the UNC Children’s Complex Care Program. Enrolled CMC will be part of the RN-clinician’s panel of patients, and they will be followed longitudinally as a distinct population. The Nurse Coordinator will serve as the organizer and a primary point of contact for enrolled CMCs’ care management needs during a one-month period following a hospital discharge.
Clinical Services- Health Information Management
The second co-primary responsibility of the Nurse Coordinator will be to implement, oversee, and evaluate a multi-pronged care management strategy for the Children’s Complex Care Program, in partnership with the Medical Director. The individual in this position will work closely with the Complex Care team to develop a strategic and operational plan as well as goals to guide implementation of the care management program.
Medical- Patient Care/Nursing
Provides care management and care coordination to enrolled children with medical complexity (CMC). Essential specific tasks may include: Direct contact with hospitalized patients/families to identify care management and care coordination gaps, with particular attention to anticipated gaps during the transitions of care timeframe (discharge to first 30 days post-discharge). Address medication issues during and after transition, including availability and administration concerns, handle refill requests, medication reconciliation, and prior authorizations as needed. Also address medical equipment and supply needs, including communication with durable medical equipment and supply companies, and work with coordinators in other specialties to meet these needs efficiently. Examples of specific gaps include re-enforcement of patient/family teaching using teach-back methods, organize and coordinate patient-centered scheduling of follow-up appointments, and arrange for post-discharge telehealth home visit (for eligible patients). Be available via pager or phone to enrolled patients/families to help troubleshoot post-discharge clinical problems.

Research- Project Management
Continuously evaluates and identifies potential quality issues and proposes/develops process improvement strategies to enhance the effectiveness of the UNC Children’s Complex Care team. Collaborates with the Medical Director to develop and implement a comprehensive quality improvement process.
Other
Work with Medical Director to plan and lead weekly Complex Care team meetings. Collaborate with existing team members (e.g., Social Worker, Administrative Coordinator, Physicians, Nurse Practitioner) to fill anticipated gaps. Instead of duplicating work, the goal is to add value by augmenting the work of existing team members.
Minimum Education and Experience Requirements
Licensed to practice as a Registered Nurse in the State of North Carolina and two years of experience in the area of specialization.
Required Qualifications, Competencies, and Experience
Work requires the analytical skills to resolve highly complex problems requiring the application of clinical, scientific or technical principles, theories and concepts and in depth, cross-functional experience-based knowledge. Ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations. Experience and comfort with common computer software and functionality: use of electronic health records, creation and sharing of spreadsheets and other tracking tools. Ability to work with patients and families from diverse cultural and linguistic backgrounds. Ability to assess and triage high risk patients.
Licensed to practice as a Registered Nurse in the State of North Carolina.
Preferred Qualifications, Competencies, and Experience
The preferred candidate will have knowledge in nursing. Experience managing transitions of care, home health, or community-based experience; knowledge of applicable federal, state, and local regulations; management and quality improvement experience.

Licensed to practice as a Registered Nurse in the State of North Carolina.
