Job Summary:

The Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) functions as part of the Texas Home Visiting team, which serves pregnant and new mothers in the perinatal period. The LCSW or LPC serves as a therapist for women enrolled in the home visiting program in Smith and Henderson Counties who have been assessed as experiencing perinatal depression and/or perinatal anxiety. The therapist receives referrals from the Nurse Home Visitors and further assesses those referred for therapeutic need. The therapist works closely with the Nurse Home Visitors to screen and identify participating women in need of intervention to be available to the role of parent of vulnerable infants.

The position meets weekly with women who are voluntarily participating in therapy and also meets regularly with the referring home visitors to coordinate therapy. The LCSW or LPC also conducts monthly Wellness Groups for all women enrolled in Nurse-Family Partnership.

This position is grant-funded. 

Therapy will be delivered in the client’s home, site of her choice, or virtually. The position can require travel throughout Smith and Henderson Counties.

Major Responsibilities / Duties / Critical Tasks:

·         Use cognitive behavioral therapy and other therapeutic modalities to treat depression and anxiety.

·         Use screening tools that identify therapeutic needs.

·         Stay educated on the evidence-based Nurse-Family Partnership program and the issues facing the pregnant and postnatal women with whom they work.

·         Deliver client-centered, well-established treatments for pregnant women and new mothers virtually or in-home.

·         Design, promote, and facilitate monthly wellness groups to address issues faced by home visiting clients to promote healthy behavior and families.

·         Maintain timely and accurate chart entries for Texas Home Visiting.

·         Identify concerns and make referrals to other mental health specialists as needed when services are outside of the scope of practice that client needs.

·         Function as a member of an interdisciplinary team to address the physical and behavioral health needs in the best interest of the infant child.

·         Handle confidential and/or sensitive information with discretion and adherence to HIPAA rules.

·         Participate in regular team meetings.

·         Participate regularly in supervision.

·         Perform other duties incidental to the work, such as attending professional conferences, training, and community coalition meetings.

·         Track, prepare, and submit travel log for reimbursement of mileage accrued in home visits.

·         Complete other duties as assigned.

Accompanying Knowledge, Skills, Abilities and Competencies:

·         Knowledge of cognitive behavioral therapy and other therapeutic modalities for treatment.

·         Knowledge of maternal health, in particular treatment of perinatal depression and anxiety.

·         Knowledge of available social resources to address social determinants of health in the two-county service area.

·         Skilled in delivering therapy with cultural humility.

·         Skilled in facilitation of groups.

·         Skilled in conducting therapy in-person or virtual.

·         Ability to work as part of a team.

·         Ability to conduct assessments to determine acuity and the most appropriate modality for treatment.

·         Competent in charting and data entry.

Competent in using Microsoft Work products.



Qualifications

 

Required Education / Experience:

·         Master’s degree in Social Work required.

·         Licensed in the State of Texas.

·         Knowledge and experience with motivational interviewing.

·         Bilingual in English/Spanish preferred.
