Position Summary
The Linkage and Retention Assistant will plan, organize, conduct, and evaluate specific HRSA, Ryan White and clinic funded activities for the Infectious Disease Clinic. The range of duties includes but is not limited to: Linking persons recently diagnosed or living with HIV to clinical care and support services, completing assessments to determine risk for poor engagement, conducting interventions that promote retention in care, assisting patients in re-engagement in care which may include facilitating transportation, scheduling, or referrals to supportive services. The Linkage and Retention Assistant will work collaboratively with the ID clinic social work team, clinic providers, state bridge counselor and project staff. They are expected to document patient encounters in the medical record and patient reporting systems. They may assist the UNC Infectious Diseases Benefits Team with administrative and scheduling duties such as: scanning sensitive documents, calling patients to schedule appointments for medication assistance applications, and data entry. The Infectious Diseases Clinic provides care and services to a diverse group of people living with HIV/AIDS. The applicant must have the ability to communicate effectively and comfortably with diverse populations.
The selected applicant will be able to improve their professional skills in the areas of quality improvement, linkage and retention in medical care, patient satisfaction, addressing barriers to care, and multi-disciplinary collaboration. The Linkage and Retention Assistant will also assist in identifying data discrepancies as they occur.
This position currently requires a hybrid work arrangement that includes both an on-site work location at the UNC Infectious Diseases Clinic in Chapel Hill, North Carolina and a remote work location that can maintain client confidentiality. This position is for 20-30 hours per week.
Minimum Education and Experience Requirements
Bachelor’s degree; or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.
Required Qualifications, Competencies, and Experience
* Strong organizational skills and attention to detail.
* Proficiency in Microsoft Word and Excel
* Excellent communication skills in person, by phone, written language and other systems such as medical record portal or texting.
* Must protect patient confidentiality under the Health Insurance Portability and Accountability Act (HIPAA).
* Ability to work independently.
* Ability to manage time effectively and achieve workplace goals.
* Ability to be empathetic to the feelings of patients and socioeconomic backgrounds.
* Knowledge of effective communication and motivation practices with diverse population. (replace the other line)
Preferred Qualifications, Competencies, and Experience
* Knowledge of HIV population needs preferred
* Bilingual (Spanish / English) preferred.
* Basic knowledge of research principles including but not limited to data collection, entry, and summary to complete defined assignments preferred
* Experience with electronic data entry, tracking and reporting and use of databases preferred.
* Priority given to applicants with experience in EPIC or EHR/EMR.
* Ability to speak with clinical staff and medical providers comfortably about patient care preferred.
* Prior case management experience preferred.
