Job Summary:

The Health Education Coordinator I (HEC I) will be working primarily on the ACCESS Cervical Cancer Prevention project funded by the Cancer Prevention Research Institute of Texas (CPRIT). The HEC I also works with the DEFEAT Breast Cancer team on the planning and implementation of outreach efforts, coordination of patient navigation services, and scheduling of patients for applicable screening examinations. The HEC I will implement outreach and inreach (for patients who are receiving care at UTHCT and our partner clinics) educational activities on cervical (and breast) cancer prevention and offers patient navigation services to facilitate scheduling and completion of applicable screening exams. This position involves an extensive community engagement and outreach efforts that includes but not limited to working closely with relevant stakeholders, fostering existing partnerships, identifying new collaborators, and working with medically underserved rural communities in East Texas to expand access to screening services and ultimately reduce health disparities.

 

Major Responsibilities / Duties / Critical Tasks:

·       Plans and delivers community outreach and inreach activities to educate the community about the importance of cancer prevention screening exams, screening guidelines, and available services.

·       Provides patient navigation services to facilitate access to screening services, help with scheduling and completion of screening as well as diagnostic exams as indicated for the target population.

  • Participates in the development of program timelines and documents progress towards meeting program milestones, metrics, and deliverables.
  • Supports a cohesive team effort to accomplish approved goals, milestones, and metrics.
  • Works with the DEFEAT Breast Cancer team on community outreach and patient navigation activities.
  • Assists in identification of new clinical partners in the community.
  • Promotes ECHO and other provider training opportunities on cervical cancer prevention delivered by the MD Anderson team.
  • Works closely with the MD Anderson team on the planning and implementation of project related activities.
  • Attends and actively participates in the patient navigators call with the MD Anderson team.
  • Gathers information to identify community needs for planning, implementing, and evaluating outreach activities designed to promote cancer prevention activities.
  • Attends regular project meetings with the DEFEAT and ACCESS team as well as meetings with the MD Anderson team.
  • Documents accurate and complete data on outreach and patient navigation services provided. 
  • Maintains accurate and complete documentation of screened patients, exam results, and diagnostic examinations as indicated.
  • Assists in the compilation of monthly reports to the MD Anderson Team and other summary reports as needed.
  • Identify and creatively designs items needed for community outreach activities.
  • Assists in developing marketing materials and social media messages tailored to reach our target population.
  • Maintains confidential records /sensitive patient information with discretion and adherence to HIPAA rules.
  •  Promotes the UT Tyler HSC mission, vision, and values by action.
  • Undertakes other duties as assigned by the Principal Investigator (PI).

  • Experience in organizing community-based events and stakeholder engagement is preferred.
  • Knowledge of EPIC is a plus.
  • Knowledge of Spanish (Bi-lingual) is preferred.
  • Community Health Worker (CHW) certification is a plus.


Qualifications

 

Required Education / Experience:

Minimum Education: High School Diploma or GED.

Preferred Education:

Bachelor’s degree in Public Health, community Health, Health Sciences, or related field preferred.

 

Minimum Experience:

Three (3) years of relevant experience in customer service, patient navigation, or other community-oriented programs in diverse settings. May accept a combination of education and experience.

Preferred Experience:
Experience working with diverse stakeholder groups and communities.

Onsite presence:

Full time in-person position that involves a considerable amount of community outreach activities. There will be a considerable amount of local travel and driving within the target counties. The position also requires working on weekends and after hours as needed. 

Working Environment:

This is an administrative/clerical environment with community outreach activities. The position requires travel to several counties in Northeast Texas to educate the community and engage stakeholders for at least a third of the time. The position will also involve working after hours and on weekends as needed to attend community-based events such as health fairs.
