Job Summary:

Nature of Work:

The University of Colorado, College of Nursing, is seeking a Family Nurse Practitioner (FNP) / Instructor / Senior Instructor / Assistant Professor (Open Rank) in the Non-Tenure Clinical Practice Track for a part-time (0.80 FTE) or full-time (1.00 FTE) benefits-eligible position at Sheridan Health Services, a Federally Qualified Health Center located in southwest Denver, CO (zip code: 80236). This position will provide integrated primary care at both our Family Health Clinic and Youth Health Clinic, serving both pediatric and adult populations.  This position includes an Instructor / Senior Instructor / Assistant Professor (Open Rank) level, non-tenure clinical practice track appointment within the College of Nursing.

Clinic Description:

SHS is a multi-site, nurse-managed FQHC, funded through the Health Resources and Services Administration Bureau of Primary Healthcare; and is a collaborative effort between the College of Nursing and the Sheridan School District. There are now 62 employees at three convenient locations to serve community health needs primarily for residents of Arapahoe, Denver, Adams, and Jefferson counties.  The CU Youth Health Clinic is located in the Sheridan School District Administration building in Englewood, CO. The CU Belleview Point Health Clinic is located in southwest Aurora and the CU Family Health Clinic is located in southwest Denver on the Fort Logan campus and provides services to adults and children, 65% of whom identify as Latinx, and 42% of whom are best served in another language, with Spanish being the predominant second language.  Services offered include primary care across the lifespan; prenatal and postpartum, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance abuse treatment.  SHS is a Patient-Centered Medical Home, and as such, we uphold a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. This position is focused on treating all patients across the lifespan at the Family Health

 Clinic at Sheridan and the Youth Health Clinic at Sheridan High School.  

Examples of Work Performed:

  • Provides acute and chronic primary care for the clinic’s Adult and Pediatric populations across the lifespan
  • Performs the full scope of Nurse Practitioner duties, to include comprehensive physical examination, diagnosis, evaluation & treatment of both chronic and acute illness. Refers to specialists when indicated.
  • Strives to provide Culturally & Linguistically Appropriate Services to patients from diverse backgrounds in an underserved, resource-limited setting
  • Coordinates patient care needs with other Care Team members, to include RN Case Managers, Referral Coordinators, Behavioral Health Team members and Pharmacy Team members.
  • Provides individualized client teaching and counseling utilizing current, evidence-based guidelines.
  • Provides health education to patients, students, families, and/or community health partners
  • Collaborates on Quality Improvement Initiatives under the direction of the Clinical Director
  • Improves and supports care delivery both within the service line and clinic-wide, by participating in programmatic planning, Team-based huddles and integrated or all-staff meetings as requested by the Leads or Clinical Director
  • Implements and upholds evidence-based Policies & Procedures
  • Functions as a clinical preceptor and/or clinical instructor for health professions students.
  • Participates in after-hours call in rotation with the other Providers on the team
  • Participates in Extended Hours (evening hours until 8pm) in rotation with other Providers on the team
  • Uphold compliance with University and professional policies/procedures.
  • Maintains Electronic Health Record documentation in compliance with accepted standards, in keeping with the Policies & Procedures of the clinic. Must meet rigorous standards for documentation, and complete notes, referrals and follow-ups in a timely manner.
  • Performs other duties as assigned.
  • Maintains all necessary licenses, certifications, credentialing materials and other required documents as outlined by the Office of Clinical and Community Affairs at the College of Nursing.

Instructor:

  • Graduate degree from an accredited school of nursing
  • Current Board certification as an Family Nurse Practitioner (FNP)

o   Current RN and APN licensures in good standing (any state) Could be working toward CO clinical licenses during application process and registered with DORA-Department of Regulatory Agencies.  Must have CO licensures by date of hire.

  • Current Prescriptive Authority (RxN) Licensure (Any State)

o   Must have Colorado RxN license by date of hire.

  • One year advanced practice experience 

Senior Instructor:

  • Graduate degree from an accredited school of nursing
  • Current Board certification as an Family Nurse Practitioner (FNP)

o   Current RN and APN licensures in good standing (any state) Could be working toward CO clinical licenses during application process and registered with DORA-Department of Regulatory Agencies.  Must have CO licensures by date of hire.

  • Current Prescriptive Authority (RxN) Licensure (Any State)

  • Demonstrated experience collaborating with academic colleagues and students in practice activities. 

Conditions of employment:

  • Must be willing and able to obtain Colorado RN and APN licenses by date of hire.
  • Must be willing and able to obtain Colorado RxN license by date of hire.
  • Must be willing and able to obtain valid NPI number by date of hire
  • Must be willing and able to obtain valid Colorado Medicaid Provider number by date of hire
  • Must be willing and able to complete CAQH by date of hire
  • Must be willing and able to obtain current BLS/CPR certification by date of hire.
  • Must be willing and able to actively participation in health plan contracts.
  • Must be willing and able to remain credentialed by the health plans and ensure credentialing materials stay current.
  • Must be willing and able to work in a normal patient care environment with some exposure to biological hazards and infectious diseases.
  • Must be willing and able to work weekends and holidays as required. 

Preferred Qualifications:

  • Experience utilizing bilingual skills (Spanish/English) at a conversation level in a clinical setting
  • Current DEA licenses
  • Experience working with population based outcomes
  • Experienced clinical preceptor
  • Experience working with marginalized populations 

Competencies:

  • Considerable degree of independent judgment and autonomy in practice.
  • Clinical expertise in both pediatric and adult primary care, both chronic and acute conditions. 
  • Willingness and ability to work in an integrated care setting, with a high degree of teamwork and collaboration
  • Ability to work until 8pm on extended hours evenings in rotation with other providers
  • Ability to use computer and clinic software, and to document in the Electronic Health Record (EHR) with thoroughness and accuracy.
  • Exceptional interpersonal and communication skills to work effectively with patients, their families, and other members of the health care team
  • Excellent knowledge of and ability to perform clinical teaching
  • Ability to effectively develop and promote practice through participation in marketing and patient recruitment
  • Ability to take one week-at-a-time on-call in a rotation schedule.
  • Availability to work evenings and weekends as needed.
  • Ability to handle average nurse practitioner caseload

Anticipated Pay Range:

The starting salary range (or hiring range ) for this position has been established as HIRING RANGE:  

·         $90,414 - $101,716 annually at Instructor of Clinical Practice/FNP

·         $90,414 - $101,716 annually at Sr. Instructor of Clinical Practice/FNP

·         $90,414 - $101,716 annually at Assistant Professor of Clinical Practice/FNP

The above salary range (or hiring range ) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position is not eligible for overtime compensation unless it is non-exempt.
