Position Summary:

The Kidney Transplant Quality Coordinator is responsible for ensuring the highest standards of quality and safety in kidney transplant programs. This role involves coordinating various aspects of the transplant process, from pre-transplant evaluations to post-transplant follow-up care, to ensure compliance with regulatory requirements and best practices. The coordinator collaborates with multidisciplinary teams, including transplant surgeons, nephrologists, nurses, and administrative staff, to optimize patient outcomes and satisfaction.

Pre-Transplant Evaluation: Coordinate the evaluation process for potential kidney transplant recipients, ensuring timely completion of medical, psychological, and social assessments.

Donor Evaluation: Facilitate the evaluation of potential kidney donors, including living and deceased donors, in collaboration with transplant surgeons and nephrologists.

Regulatory Compliance: Ensure compliance with regulatory standards and guidelines set forth by regulatory bodies such as UNOS (United Network for Organ Sharing) and CMS (Centers for Medicare & Medicaid Services).

Quality Improvement Initiatives: Develop and implement quality improvement initiatives to enhance the effectiveness and efficiency of kidney transplant programs.

Data Management: Maintain accurate and comprehensive records of transplant activities, including patient evaluations, transplant surgeries, and post-transplant outcomes.

Patient Education: Provide education and support to kidney transplant candidates and recipients regarding pre- and post-transplant care, medication management, and lifestyle modifications.

Multidisciplinary Collaboration: Collaborate with transplant surgeons, nephrologists, nurses, social workers, and other healthcare professionals to ensure coordinated and comprehensive care for transplant patients.

Outcome Monitoring: Monitor transplant outcomes, including graft survival rates, patient survival rates, and complication rates, and identify areas for improvement.

Patient Follow-Up: Coordinate post-transplant follow-up care, including monitoring of graft function, medication adherence, and management of complications.

Continuous Education: Stay informed about advancements in kidney transplant care, emerging best practices, and changes in regulatory requirements through ongoing education and professional development activities.


Position Qualification:

Bachelor's degree in nursing, healthcare administration, or related field (Master's degree preferred)

Registered Nurse (RN) or other healthcare licensure/certification as required by state regulations

Experience in transplant coordination or related healthcare role preferred

Strong knowledge of regulatory requirements and best practices in kidney transplant care

Excellent communication, interpersonal, and organizational skills

Ability to work effectively in a multidisciplinary team environment

Proficiency in data management and analysis

Commitment to quality improvement and patient-centered care
